摘  要:目的探讨诱导型一氧化氮合酶(iNOS)在腹部肠管火器伤后心脏中表达的变化及其意义。方法健康长白仔猪42头随机等分为对照组和腹部肠管火器伤后1、2,4、8、12和24h实验组。用免疫组化图像分析法测定各组心肌组织iNOS的表达,同时测定血清LDH、CK—MB水平,在光、电镜下观察各组心肌组织学变化。结果伤后各组心脏iNOS表达明显高于对照组。伤后各组血清LDH、CK—MB水平均高于对照组。伤后8、12、24h组光镜下出现逐渐加重的心肌细胞水肿、变性;电镜下4、8、12、24h组出现逐渐的线粒体肿胀、溶解;对照组光、电镜下未见明显的损伤性变化。结论腹部肠管火器伤后心脏iNOS表达增强,与心脏结构和功能损伤基本一致;iNOS的活化在腹部火器伤肠管穿透后继发性心脏损伤中扮演着重要角色。Objective To investigate the expressions of iNOS and its significance in injury heart after intestinal firearm wound. Methods A total of 42 Chang - bai piglets were randomized into 7 groups: control group and wounded 1 h group, 2 h group, 4 h group, 8 h group, 12 h group, 24 h group. The model of intestinal firearm wound were established in wounded groups. Cardiac iNOS expressions were measured with immunohistochemical staining and image analysis in all groups. Levels of serum LDH and CK - MB were determined. The histological change of cardiac tissue were observed with light microscope and electron microscope (EMS). Results Levels of cardiac iNOS expression in wounded groups were significantly elevated compared to control group(P 〈 0.05), so were the levels of serum LDH and CK - MB. The experimental group of 8 h, 12 h, 2g h appeared gradually aggravated cardiac cells edema and degeneration under light microscope, mitochondrial appeared gradually aggravated swelling and dissolved under EMS in 4 h, 8 h, 12 h, 24 h group. There were no obviously change in the injury in the control group under light microscope and EMS. Conclusion After intestinal perforations due to abdominal firearm wound, cardiac iNOS activity increases and it is positive correlation with cardiac morphological and functional injury; iNOS can play an important role in subsequent heart injury after the wound.
