摘　　要：目的回顾性分析12例肺硬化性血管瘤(pulmonary sclerosing hemangioma,PSH)的临床病理及免疫表型特点。方法收集12例PSH患者的临床及病理资料,在光镜下分析其组织学结构,应用免疫组织化学SP法检测CK、EMA、TTF-1、Syn、CgA、CR、Ⅷ因子、ER、PR、vimentin。结果PSH由实性结构、乳头状结构、硬化性结构和血管瘤区组成。主要的两种细胞为立方状表面细胞和圆形间质细胞。立方状表面细胞表达广谱CK、TTF-1、EMA,圆形间质细胞表达EMA、TTF-1、ER、PR、vimentin;两种细胞均无间皮及血管内皮标记的表达,个别病例两种细胞不同程度散在表达Syn、CgA。结论PSH是一种肺内少见的肿瘤,组织学形态呈多样性,主要以混合型结构为主。免疫组织化学染色在诊断与鉴别诊断中有重要作用。Purpose To analyze retrospectively the clinicopathological characteristics and immuophenotype in 12 cases of pulmonary sclerosing hemangioma（PSH）.Methods Twelve cases of PSH were investigated by light microscopy.CK,EMA,TTF-1,Syn,CgA,CR,Factor-Ⅷ,ER,PR and vimentin were detected by SP immunohistochemistry.Results Pathological morphology demonstrated cuboidal cells and polygonal cells composed four types of migratory structures,including papillary,solid,sclerotic and hemorrhagic pattern.Immuniohistochemistry results showed that cuboidal cells of PSH expressed CK（Pan）,EMA and TTF-1 and polygonal cells of PSH expressed EMA,TTF-1,ER,PR and vimentin.Neither of these two types of cells expressed mesothelial and vascular endothelial markers.Sporadic expression of Syn and CgA was observed in a few cases.Conclusion PSH is a rare lung tumor,with various histologic appearance.The mixed-type is more than other types.Immuniohistochemistry plays an important role in the diagnosis and differential diagnosis.
