摘  要:目的 介绍一种能扩大手术视野,可在反射直视下完成下颌角截骨手术的新型口腔颌面拉钩的制作及使用方法.方法 以传统颌面拉钩为基础,研制一种带镜面、光导纤维管的新型镜面光导口腔颌面拉钩,以动物实验为基础,比较传统颌面拉钩、镜面光导拉钩在下颌骨截骨术中的可视状况、手术时间、出血量、双侧截骨的准确性和对称性.结果 应用新型镜面光导口腔颌面拉钩经口内入路行下颌角手术时可实现对手术区域的直视操作.与传统拉钩比较,手术时间明显缩短(P〈0.01),出血量少(P〈0.01);对照组与实验组比较,双侧截骨的长宽差异有统计学意义(P〈0.05).结论 该器械结构简单,操作简便;利用镜面反射扩大手术视野,提高了手术操作的精确性,缩短了手术时间,保证了手术质量,减少了手术并发症.Objective To introduce the manufacture and initial experimental study of light guiding oral retractor by slickenside which could expand operative field of vision for the mandible osteotomy under reflect orthophoria. Methods Based on the traditionally maxillofacial retractor, a neotype oral retractor with slickenside and optical fiber was manufactured. Compared with the traditionally maxillofacial retractor, mandible osteotomy was performed in animal models with the manufactured maxillofacial retractor, and the relative data of operating field of vision, operating time, bleeding volume, accurate and symmetry of bilateral mandible osteotomy were collected. Results Through the intraoral approach, mandibular angle osteotomy could be performed under orthophoria by using the neotype retractor. Compared with the traditional retractor, operating time was shorter ( P 〈 0.01 ), bleeding volume was less ( P 〈0. 05), and there was statistical significance in length and width of osteotomy ( P 〈 0.05) between the experimental and control groups. Conclusion The neotype retractor is simple in structure and operation. It can expand the operating field through the mirror reflection which makes the operation more delicate and faster, and reduce the risk of complications.
