氟尿苷碘化油乳剂在巨大肝细胞癌介入治疗中的应用

摘　　要：目的探讨氟尿苷碘化油乳剂在巨大肝细胞癌(hHCC)经导管动脉化疗栓塞(TACE)治疗中的应用价值。方法选择hHCC患者32例,经导管向肿瘤血管内注入氟尿苷碘化油乳剂,后续加用明胶海绵颗粒封闭肿瘤供血动脉。术后1个月行肝脏CT或MR检查,观察肿瘤大小变化;比较术前及术后1个月血清AFP的变化。结果32例患者手术全部获得成功,共接受TACE治疗98次,每例患者最多治疗6次,最少1次,平均3.1次。碘化油平均用量16 ml。术后1个月复查CT或MR示,有效28例(87.50%),稳定3例(9.38%),进展1例(3.12%)。血清AFP值由术前(1235.26±76.18)μg/L下降至术后(429.31±21.67)μg/L(P<0.05)。结论氟尿苷碘化油乳剂TACE治疗hHCC效果明确,患者无严重不良反应,比较安全,具有可行性。Objective To explore the value of Floxuridine-lipiodol in the transcatheter arterial chemoembolization（TACE） for huge hepatocellular carcinoma（hHCC）.Methods Thirty-two patients with hHCC received TACE with Floxuridine-lipiodol,then the feeding artery was blocked using gelatin sponge.One month later,CT and MR were performed to observe the tumor size and the level of AFP.The tumor size and AFP were compared before and 1 month after operation.Results All of the 32 patients received TACE successfully.A total of 98 times TACE were performed,each patient received 1 to 6 times TACE,with the average number of 3.1 times.The average volume of iodized oil was 16 ml.CT and MR proved that the lesion volume shrinked in 28 patients（87.50%）,stayed stable in 3 patients（9.38%） and progressed in only 1 patient（3.12%）.The level of AFP descended from（1235.26±76.18）μg/L to（429.31±21.67）μg/L（P〈0.05）.Conclusion The effect of Floxuridine-lipiodol in TACE of hHCC is identified.TACE with Floxuridine-lipiodol is safe and feasible.
