摘 要:目的观察特发性多发性肌炎患者外周血淋巴细胞共刺激分子CD28、CD152、CD80、CD86表达,进一步分析共刺激分子与疾病的关系。方法研究分三组,即健康对照者(HC,n=5)、特发性多发性肌炎组(IPM,n=8)和肌营养不良组(MD,n=8),后两组患者均经肌肉活检酶组织化学确诊。三组患者均通过流式细胞术检测外周血淋巴细胞CD4、CD8、CD4+CD28+、CD8+CD28+、CD152、CD80、CD86表达,并计算出CD4+CD28-、CD8+CD28-表达。结果与HC组比较,IPM组CD4+CD28+表达降低(31.9±6.6 vs 39.5±6.9,P〈0.05),其所占CD4+T细胞的百分率亦明显下降(92.2%±6.2% vs 98.1%±0.7%,P〈0.05),与之相反的是CD4+CD28-表达明显升高(2.0±1.8 vs 0.6±0.2,P〈0.05)。IPM组CD4、CD8表达及CD8+CD28+/CD8百分比与HC组无明显差异,CD8+CD28-表达则呈上升趋势。IPM组CD152较HC组明显升高(18.5±13.8 vs 7.1±1.2,P〈0.05),各组间CD80表达无明显差异,CD86以IPM组最高(6.8±1.9 vs 4.6±1.7,P〈0.05)。结论IPM患者外周血CD4阳性的T细胞表面的CD28表达下降,但CD152和CD86表达升高。Objective To investigate the expression of CD28, CD152, CD80 and CD86 in peripheral blood lymphoeytes in idiopathic polymyositis, and determine the relationship between these costimu- latory molecules and this disease. Methods Clinical data of healthy controls (HC) ( n = 5), patients with idiopathic polymyositis (IPM) ( n =8), muscular dystrophy (MD) ( n =8) were collected. Mus- cular biopsies were carried out on all the patients. The expression of costimulatory molecules CD4+ CD28+ , CD8+ CD28+ , CD152, CD80 and CD86 in peripheral blood lymphocytes were detected by flow cytometry (FCM). The expression of CD4+ CD28- and CD8+ CD28- were calculated. Results Compared with HC group, the expression of CD4 + CD28 + Tlymphocytes were decreased in IPM group (31.9±6.6 vs 39.5±6. 9, P 〈 0. 05), and the same pattern was observed in the expression of CD4+ CD28+/CD4 (92.2%±6.2% vs 98.%±0. 7%, P 〈 0. 05). In contrast, the expression of CD4+ CD28- increased significantly in IPM group compared with HC group (2.0±1. 8 vs 0.6±0.2, P 〈 0.05). There was no difference in the expression of CD4, CD8 or the percentage of CD8+ CD28+/CD8. However, the expression of CD8+ CD28- had a slight increase. The expression of CD152 in IPM group was higher than in HC group (18.5±13.8 vs 7.1±1.2, P 〈 0. 05). No differenee in the expression of CD80 was found in the three groups, whereas the expression of CD86 was highest in IPM group (6.8±1.9 vs 4.6± 1.7, P 〈0.05). Conclusions The expression of CD28 in peripheral blood CD4 positive T lymphoeytes decreased, whereas the expression of CD152 and CD86 increased in idiopathic polymyositis.
