CT引导下三叉神经半月节射频热凝治疗原发性三叉神经痛临床观察

摘　　要：目的 评价CT引导下三叉神经半月节射频热凝治疗原发性三又神经痛的临床效果.方法 选取原发性三叉神经痛患者30例,经CT定位引导下向患侧卵园孔穿刺至三叉神经半月节,给予阻抗、神经电生理测试后,先行42℃、60 s的自动脉冲射频治疗,再给予80℃、90s,3个周期的射频热凝,术中监测血压、心率、脉搏血氧饱和度.结果 30例原发性三叉神经痛患者经本法治疗后,27例患者疼痛立即消失,随访2～15个月无复发;1例术后1年疼痛复发,1例术后疼痛减轻,另1例完全无效,均无重大并发症发生.结论Objective TO evaluate the clinicat efficacy of CT-guided radiofrequency thermocoagulation of trigeminal semilunar ganglion for treatment of primary trigeminal neuralgia （PTN）. Methods Thirty patients with PTN were included in the study. Under CT positioning and guidance,the trigeminal semilunar ganglion was punctured through the oval foramen.When the resistance and electrophysiological tests were normal,the first auto-pulsed radiofrequency therapy of 420（3 for 60 seconds was performed,then second three-course radiofrequency thermocoagulation of 80℃ for 90 seconds were followed under the monitoring of blood pressure, heart rate, pulse and SpO2. Results Among 30 patients, the pain was instantly relieved in 27 after radiofrequency thermocoagulation,there was no recurrence during the follow-up for 2 - 15 months; 1 patients recurred after 1 year, 1 patient was partially relieved and 1 patient was unchanged. No serious complications occurred in all the patients. Conclusion CT-guided radiofrequency thermocoagulation of trigeminal semilunar ganglion is an effective therapy for difficult cases of trigeminal neuralgia.
