角膜地形图及波前像差仪观察原位角膜磨镶术后像差的改变

摘　　要：目的探讨角膜地形图仪检测LASIK手术前眼散光及手术切削散光的差异;应用傅立叶分析瞳孔区3.0 mm、6.0 mm直径光学区像差在手术前后的变化及对术后视力的影响,及波前像差仪观察的像差改变与角膜地形图仪观测结果是否一致。方法选择LASIK手术后24例复诊病人48眼,术后19眼裸眼视力1.0,29眼裸眼视力1.2;本组所有眼均行角膜地形图检查,Fourier（傅立叶）分析直径为3.0 mm、6.0 mm光学区像差。结果手术切削散光值差异有统计学意义,但散光轴变化不明显。术后3.0 mm、6.0 mm区像差均增加。高阶像差、Astigmatism、Coma术后表现为减少并且差异有统计学意义,Trefoil、sph.aberration手术前后无变化。2-6阶的Zernike系数的改变表现为2、3、4、6阶均表现为减低,且差异有统计学意义。结论LASIK手术角膜散光的切削变化不显著。而高阶像差改变不对术后视力产生影响。波前像差与角膜地形图结论不同。Objective To discuss the difference of astigmatism before LASIK and after remotion and analyze optics of pupil area before and after operation with Fourier and its affection of sight after operation as weel as changes with corneal topography and wavefront astigmation.Methods 48 eyes of 24 cases after LASIK were detected with corneal topography whose naked vision（19 eyes 1.0 and 29 eyes 1.2）.The astigmation was analyzed with Fourier of diameter 3.0 mm and 6.0 mm.Results There was significant difference of remotion astigmation with LASIK but its axis was not changed obviously.The astigmation of 3.0 mm and 6.0 mm area were increased.The high level astigmation,astigmatism and coma were decreased after operation with significance but trefoil adsh.aberration.The zernike index changed decreased at 2-6 grade with significance.Conclusion The LASIK had less affection on astigmation remotion.The high level astigmation was not affected vision after operation.The conclusion of wavefront astigmation and corneal topography might be differece.
