双颌手术联合术后快速正畸矫治唇腭裂术后严重双颌畸形

摘　　要：目的:探讨应用双颌手术联合术后快速正畸建矫治唇腭裂术后严重双颌畸形的方法。方法:2002年1月～2006年4月,共收治唇腭裂术后严重双颌畸形患者18例(男8例,女10例),年龄16～33岁,平均24.5岁。单侧唇腭裂15例,双侧3例,所有患者术前均未接受过正畸治疗,均联合应用双颌手术和术后快速正畸建矫治唇腭裂术后严重双颌畸形。结果:18例患者手术后效果良好,复查头颅X线片,上、下颌骨位置均达到术前设计位置。手术前后头影测量结果(SNA角、SNB角,ANB角)t检验,具有显著性差异(P<0.05)。经过3～8个月快速术后正畸治疗,均建立良好的关系,恢复正常咬合功能。随访3～7年,效果稳定。结论:唇腭裂术后严重双颌畸形采用双颌手术联合术后快速正畸,不但能够显著地改善双颌畸形,而且可以快速建立正常咬合关系。Objective To investigate an effective method of simultaneous bimaxillary orthognathic surgery and rapid orthodontics postoperation for correction of secondary severe bimaxillary deformities in cleft patients.Methods From January 2002 to April 2006,18 cleft patients suffering from secondary severe bimaxillary deformities were treated.8 males and 10 females,aged from 16 to 33 years（mean age：24.5 years）.15 patients were unilateral cleft and 3 were bilateral cleft.All patients hadn＇t received orthodontics preoperation and were treated by bimaxillary orthognathic surgery accompanied with rapid orthodontics postoperation for deformities correction.Results All 18 patients were satisfied with their appearances and dental articulation after operation.With an X-ray re-examination,maxillary and mandibular bone were returned to their normal position in all patients.Angles of SNA,SNB and ANB of X-ray cephalometry were measured,In statistical analysis,the correlation of angles between before and after operation were statistically significant（t test;P0.05）.All patients got the normal occlusion and the satisfactory intermaxillary relationship with rapid orthodontics treatment of 3～8 months.After a follow up of 3 to 7 years,dental arch had good appearance.Conclusion The simultaneous bimaxillary orthognathic surgery and rapid orthodontics postoperation is efficient for correction of secondary severe bimaxillary deformities in cleft patients.It can not only correct bimaxillary deformities notablely,but also establish normal occlusion rapidly.
