中药清热消炎固齿缓释剂对慢性牙周炎龈沟液中弹性蛋白酶水平的影响研究

摘　　要：目的:观察成人慢性牙周炎应用中药清热消炎固齿缓释剂治疗前后龈沟液中细胞外弹性蛋白酶EA-S(elastase in supernatant)和细胞内弹性蛋白酶EA-P(elastase in the pellet)水平的变化。方法:挑选40例慢性成人牙周炎患者,将患者的左右两颗患牙随机分为实验组和对照组,基础治疗后实验组使用清热消炎固齿缓释剂,实验时间持续4周,1周1次。对照组不使用清热消炎固齿缓释剂,最后测试对比两实验小组治疗前后龈沟液中的EA-S和EA-P水平。结果:实验组和对照组在治疗后龈沟液中EA-S和EA-P总量和浓度均明显降低(P<0.01),治疗前后实验组与对照组龈沟液中EA-S和EA-P总量和浓度变化有显著性差异,实验组优于对照组(P<0.01)。结论:基础治疗和清热消炎固齿缓释剂联合应用对成人慢性牙周炎的治疗在降低龈沟液中EA方面优于单纯的基础治疗。Objective： To investigate changes of the level of elastase in supernatant（EA-S） and elastase in the pellet（EA-P） in gingival crevicular fluid（GCF） of chronic periodontitis patients after the treatment of Qingre Xiaoyan Guchi local slow-release agent.Methods： Two involved teeth of both sides of 40 adult periodontitis patients were randomly divided into experimental group and control group.After basic treatment,the experimental group was given with extra treatment of the Qingre Xiaoyan Guchi local slow-release agents once a week for 4 weeks.Levels of EA-s and EA-p in GCF of the two groups before and after the treatment were evaluated and compared with each other.Results： Both of the concentration and total amount of EA-s and EA-p in GCF obviously declined after treatment（P〈0.01）,and the experimental group showed significant better results than that of the control group（P〈0.01）.Conclusion： Combination of basic treatment and Qingre Xiaoyan Guchi local slow-release agent can decrease the level of EA-S and EA-P in GCF of chronic periodontitis patients,more effective than the simple basic treatment.
