两种方法矫治薄角膜近视眼的临床疗效观察

摘　　要：目的观察超薄瓣准分子激光原位角膜磨镶术和准分子激光角膜上皮瓣下磨镶术两种方法矫治薄角膜近视眼的疗效,并对其临床特点进行分析比较。方法对60例（120眼）薄角膜近视患者随机平分两组,A组采用超薄瓣Lasik矫治,B组采用Lasek矫治;分别于术后1、3d和10d、1、3、6个月复诊;复查裸眼视力、最佳矫正视力、裂隙灯、眼压、电脑验光、角膜地形图等并记录患者的视觉不适症状。结果 A组症状轻微,视力恢复快;B组眼部刺激症状较重,视力恢复较慢,并有haze出现。术后3个月~半年,两组裸眼视力,残留屈光度数的差异无统计学意义。术后A、B组存在的视觉不适比例分别为9%、8.8%,相比差异无统计学意义（P=0.0527）;复查角膜地形图所有患者均没出现角膜前突征象。结论通过综合分析角膜厚度和屈光度,合理的选择超薄瓣Lasik或者Lasek能够为薄角膜近视患者特别是一些因就业急需矫治近视的患者提供安全、有效的屈光矫治。Objective To observe the effect of Lasik and Lasek in treating thin-flep corneal short-sightedness.Methods 60 cases（120 eyes）of thin-flap corneal short sightednese were selected randomly and divided into two groups.Group A was treated by Lasik,group B by Lasek.Patients of both groups were 1 day,3day,Ten day,1 month,3 months and 6 months after operation respectively.The items for examination are： uncomected visual acuily map tests eye′s discomforts were also hcorded.Results Patients of group a had no eye,s discomfortalle feeling in their visnal acuity had a very quick recoery patients of group B had much dicomfortabls feeling in their eyes and their visnal acuity had a very slow recovery.And what was more,there was haze appearing before their eyes.However,the uncorrected visnal acuity and the remaining corneal diopter of there two groups examined and comared after 3 months and half-a-year had no statiotie Rignificance.Eye′s discomfortable feeling rate of these two groups are 9% and 8.9% respectively.The compariseon has no statistie significance（P=0.0527） too.Corneal map examination of both showed no Kerato-ectasia.Conclusion Through the examination of corneal thickness and its diopter.We find that asuitable choose of Lasik or Lasek can give an effective treatment to those who for employment are eager to cury their short-sightedness.
