南京市社区居民社区首诊意愿及影响因素研究

摘　　要：目的了解居民对社区首诊制的认知、意愿及其影响因素、就医行为以及对社区首诊实施后的感受。方法采用方便抽样,对南京市某社区397名社区居民进行问卷调查,运用SPSS16.0对调查数据进行分析。结果对社区首诊概念的知晓率为72.80%,对社区首诊政策知晓率为42.57%,有69.77%的居民有社区首诊意愿,55.42%的居民首选社区卫生服务机构就诊,社区首诊实施后居民对社区卫生服务机构的总体评价变好的比例为57.43%、变差的比例为7.30%;单因素分析结果显示,文化程度、距家最近的医疗机构、对社区首诊政策的知晓、对社区卫生服务机构的技术满意度与居民是否愿意去社区卫生服务机构首诊有关联(P均<0.05);多因素分析结果显示,对社区首诊政策的知晓、对社区卫生服务机构的技术满意度是影响居民社区首诊的因素。结论社区卫生服务机构的服务能力仍需提高,社区首诊政策的宣传力度有待加强。Objective This paper aims to understand the residents＇ willingness to select community doctor as gatekeeper and its determinants,health seeking behavior and appraisal on the policy.Methods On the principle of convenience sampling,397 residents from community were investigated and data was analyzed with SPSS16.0.Results The awareness rate of the conception and the policy of community doctor as gatekeeper among 397 residents was 72.80%、42.57% respectively.69.77% of residents have the willingness to select community doctor as gatekeeper,55.42% of them visited community health facilities firstly when they were sick,57.43% of residents considered the overall impression to community health facilities become better,while 7.30% of them considered the situation become worse.The results of single factor analysis showed that education degree,the health facilities closest to residence,the awareness rate of the policy,medical technologies of community health facilities were associated with residents＇ willingness to select community doctor as gatekeeper;The awareness rate of the policy,medical technologies of community health facilities did influence residents＇ willingness to select community doctor as gatekeeper in logistic analysis.Conclusion the ability of medical service in community health facilities need be improved and the policy need propagandized widely.
