P15、P27和Bcl-2在人食管鳞状细胞癌中的表达及其临床意义

摘　　要：目的探讨人食管鳞状细胞癌组织P15、P27和Bcl-2的表达及其临床意义。方法用免疫组化及Westernblot方法检测人食管鳞癌组织中P15、P27和Bcl-2的表达,结合病例随访资料分析其临床意义。结果 48例食管鳞癌病例获得完整随访资料,鳞癌组织中Bcl-2的表达(90.5%)明显高于癌旁正常组织(6.3%)(P<0.05);高分化鳞状细胞癌组织中Bcl-2的表达(80%)明显低于低分化鳞癌(91.7%)(P<0.05);在有淋巴结转移的鳞癌中Bcl-2的表达(100%)明显高于无淋巴结转移鳞癌(83.3%)(P<0.05)。P15、P27在人食管鳞状细胞癌组织中的表达显著低于癌旁正常黏膜组织(P<0.05),而在高级别上皮内瘤变中P15、P27的表达就明显降低(P<0.05)。结论 P15、P27和Bcl-2的表达与人食管鳞癌的发生及进展有一定相关性;P15、P27表达的降低为食管鳞癌发生的早期事件,检测P15、P27和Bcl-2的表达对人食管鳞癌的早期诊断、判断预后有参考价值。Objective To study the expression and clinical significance of P27 P53 and Bcl-2 in esophageal carcinoma.Methods Immunohistochemical staining and Western blot were used to determine the expression of P15、 P27 and Bcl-2 in 48 cases of esophageal carcinoma.Analysis of their clinical significance was made in combilation with the information of patients.Results The positive incidences of Bcl-2 expression in esophageal carcinoma were 90.5%;The positive incidences of Bcl-2 expression in normal esophage were 6.3%.The rate of Bcl-2 expression was lower in normal esophage than that in esophageal carcinoma tissues（P0.05）.The positive incidences of Bcl-2 expression in well-differentiated esophageal carcinoma were 80%,and it was 91.7% in poorly differentiated esophageal carcinoma.The positive incidences of Bcl-2 expression in lymph node metastasis of esophageal carcinoma were 100%,and it was 83.3% in lymph node without metastasis.The expression of P15 and P27 was much lower in esophageal carcinoma than that in normal esophagal tissues.In high-ranking intraepithelial tumor the positive incidences of P15 and P27 was much lower（P0.05）.Conclusion P15 and P27 expression had a negative relation with expression of Bcl-2.The P15 and P27 negative expression is that it is an early incidence about development of esophageal carcinoma.Expression of P27 and P15 may prove to be valuable in treatment and prognosis assessment of esophageal carcinoma.
