摘　　要：医保经办机构作为"天然"的政府垄断性事务经办组织,随着医保制度的改革完善和"全民医保"时代的来临,其缺陷和弊端日益开始暴露出来。在医疗卫生体制深化完善的过程中,特别是伴随着医院独立法人主体地位的塑造和强化,医、保、患三者关系也面临重构。在这样一个新形势下,医保经办机构需要引入竞争机制,通过探索第三方托管、发展非营利性医保机构和鼓励跨统筹区经办等形式,打破垄断,优化经办,提升能力,构建和谐。The defects and drawbacks of medial agency,as a ＂natural＂ governmental monopolistic organization,along with the perfecting reform of medical insurance system and the coming age of ＂universal coverage of medical insurance＂ ,are exposing day by day. In the process of the perfecting reform of health care system,especially by the dominant position of hospitals＇ independent legal entities shaping and strengthening,the relationship of hospital,insurance company and patient is also confronted with reformation. Under this new condition,the medical agency needs to introduce competing mechanisms,through the forms such as exploring the escrow,developing the non-profit medical insurance institution and encouraging to operate across the coordination regions,in order to break the monopoly,optimize the operation,enhance the capacity and build the harmony.
