乙肝肝硬化患者的蛋白营养支持治疗

摘　　要：目的观察以植物蛋白为主的混合蛋白饮食对乙型肝炎肝硬化患者的营养支持作用。方法选择有低蛋白血症的肝硬化患者60例,其中蛋白饮食组(实验组)30例,对照组30例,分别给予以植物蛋白为主的高蛋白、高热量食谱和医院常规膳食,4周后观察其血清白蛋白、球蛋白、体质指数和上臂肌围等营养评价指标的变化,评价其效果。结果与治疗前相比,实验组和对照组各项指标均有显著改善。观察组血清白蛋白明显上升,球蛋白明显下降,与对照组相比有显著差异(P<0.05),2组的体质指数均有所上升,观察组上升较为明显,但无统计学差异(P>0.05),观察组的上臂肌围增加明显优于对照组(P<0.05)。结论高蛋白营养支持饮食有利于改善肝硬化患者的营养不良状态。Objective To observe the effect of nutrition support of using the mixed protein diet which mainly focuses on the vegetable protein in the patients with cirrhosis developing from hepatitis B.Methods Choose 60 patients with cirrhosis accompanying with hypoproteinemia.They are divided into two groups：protein diet group（experimental group,n=30）and the control group（n=30）.All patients are administered to high protein,high quantity of heat and the regular meal of hospital which mainly focuse on the vegetable protein.After 4 weeks,observe the changes of the nutritional targets of serum albumin,immunoglobulin,constitution index and Arm Muscle Circumference,and then assess its effect.Results Comparing with pre-treatment,significant improvements of all targets can be shown in both the experimental group and the control group.Serum albumin rises and immunoglobulin descends evidently in the observed group,which shows significant difference comparing with the control group（p0.05）.Constitution indexes rise in both two groups.It rises more obviously in the observed group but no statistic difference（p0.05）.Arm Muscle Circumferences of patients’in the observed group increase more obviously than that in the control group（p0.05）.Conclusion Nutrition with high protein is beneficial to improve malnutrition states of the patients with cirrhosis which develops from hepatitis B.
