摘  要:目的 探讨MRI"皂泡征"和"环征"对骨巨细胞瘤(GCT)的定性诊断价值.方法 分别用MRI"皂泡征"、"环征"、"皂泡征"+"环征"对200例骨肿瘤或肿瘤样病变进行GCT的预判,并与病理结果对照,其中骨巨细胞瘤有22例.结果 (1)"皂泡征":病变内≥3个的圆形"皂泡样"小囊,呈水样长T1、长T2信号,边缘光滑清晰,增强扫描无强化;"环征":病变边缘T1WI、T2WI像有完整或不完整的低信号带;(2)"皂泡征"诊断骨巨细胞瘤的敏感性为95.45%,特异性为97.75%,诊断正确率达97.5%;"环征"的诊断敏感性为100%,特异性为75.84%,诊断正确率78.5%;"皂泡征"+"环征"的诊断敏感性为95.45%,特异性为99.44%,诊断正确率达99%.结论 MRI"皂泡征"+"环征"诊断GCT的敏感性好、特异性高,具有很高的定性诊断价值.Objective To evaluate the qualitative diagnostic value of "soap bubble sign" and "ring sign" of MRI for bone giant cell tumor. Methods The diagnosis of giant cell tumors(GCT) was predicted in 200 cases of bone neoplasms or tumor--like lesions proved pathologically (including 22 cases of GCT) with "soap bubble sign" ,"ring sign" , "soap bubble sign" and in combination with "ring sign" on MR images,respectively and compared with pathological data. Results (1)The "soap bubble sign"that was over 3 round "soap bubble--like'cysts with clear margin inside the lesions, which were hypointensity on T1WI and hyperintensity on T2 WI, meanwhile no contrast--enhancement showed after Gd--DTPA injected. The "ring sign"revealed integrated or unintegrated hypointense circle. (2)In diagnosing GCT by MRI, the sensitivity, specificity and accuracy were 95.45 %,97. 75% and 97.5 % based on "soap bubble sign" 100%,75.84% and 78.5% based on "ring sign",and 95.45%, 99.44% and 99% based on both"soap bubble sign" and "ring sign" , respectively. Conclusion "soap bubble sign'combined with "ring sign" are of important value in preoperative evaluation of bone giant cell tumor.
