摘　　要：目的:调查无痛胃肠镜与常规胃肠镜的临床经济情况、安全舒适度及检查者和被检查者满意度等,为无痛胃肠镜在我国临床的选择应用提供依据.方法:2009-03/2009-09在武汉大学中南医院及中国人民解放军广州军区武汉总医院随机抽取胃肠镜检查的1800例患者和11名进行内镜操作的医生,阅读调查须知及填写问卷调查表.其中常规胃肠镜检查组患者1000例,无痛胃肠镜组患者800例.结果:常规组与无痛组患者在职业、学历、收入水平、费用来源等方面有显著性差异(P<0.01).两组不能耐受检查的比例无统计学差异(P>0.05).两组舒适性和安全性比较,差异均有显著性(P<0.01).两组患者满意度无显著性差异(P>0.05).医师认为常规胃肠镜更安全.结论:常规胃肠镜经济、安全、效果好,目前仍为大多数患者所接受.无痛胃肠镜痛苦小,但是价格较高,有一定风险,适用于不宜或不愿接受常规内镜检查的患者.进行结肠镜检查的患者更愿意接受无痛检查.AIM：To compare the clinical costs,safety and comfortableness as well as the satisfaction degree among examinees and examiners between sedated gastrocolonoscopy and conventional gastrocolonoscopy.METHODS：One thousand and eight hundred patients undergoing gastrocolonoscopy and 11 physicians conducting such examinations at either Zhongnan Hospital of Wuhan University or Wuhan General Hospital of Guangzhou Military Command from March 2009 to September 2009 were enrolled to fill out a questionnaire.Of all patients,1 000 underwent conventional gastrocolonoscopy,and 800 underwent sedation gastrocolonoscopy.RESULTS：The two groups of patients had significant differences in occupation,educational background,income level and the source of costs （all P〈 0.01）.There was no statistical difference in the number of patients intolerant of the examinations between the two groups （P 〉0.05）.Significant differences were also noted in the comfortableness and safety between the two groups （P 〈0.01）.Although there was no significant difference in patient＇s satisfaction degree between the two groups （P〉 0.05）,the physicians believed that conventional gastrocolonoscopy was safer than sedated gastrocolonoscopy.CONCLUSION：Conventional gastrocolonoscopy is economic,effective and safe,and is accepted by the majority of patients.Though sedated gastrocolonoscopy is more comfortable and less painful,it has a higher price and risk.Therefore,sedated gastrocolonoscopy is suitable for patients who are unwilling to or cannot undergo conventional endoscopy.Patients who previously underwent colonoscopy prefer to accept sedated examination.
