经腹脐带穿刺术用于产前诊断染色体异常的临床分析

摘　　要：目的:分析胎儿脐血染色体核型检测结果,评价经腹脐带穿刺术在妊娠中晚期孕妇产前诊断中的应用。方法:对312例有脐血检测指征的孕妇在B超引导下行经腹脐带穿刺术,并根据产前诊断需要行脐血染色体核型分析。结果:成功穿刺脐血306例,成功率为98.08%,均培养成功。发现染色体异常12例,异常率为3.92%,其中21-三体4例、染色体多态性3例、常染色体易位2例、18-三体1例、性染色体异常1例、染色体倒位1例。结论:在B超引导下经腹脐带穿刺术检查胎儿染色体核型具有较高的成功率,可以满足具有不同产前诊断指征,特别是妊娠中晚期孕妇的要求,是一种比较安全和可靠的产前诊断方法。Objective:To analyze the results of chromosomal karyotype detection in cord blood of fetus,and evaluate the application of transabdominal cordocentesis in prenatal diagnosis of pregnant women in the second and third trimester pregnancy.Methods:312 pregnant women with indications of cord blood test underwent transabdominal cordocentesis guided by B type ultrasound,then chromosomal karyotype analysis of cord blood was carried out according to the demand of prenatal diagnosis.Results:Transabdominal cordocentesis was succeed in 306 pregnant women,the success rate was 98.08%,and chromosomal karyotype analysis was succeed in all of pregnant women.12 pregnant women were found with chromosomal abnormality,the rate was 3.92%,including 4 pregnant women of 21 trisomy syndrome,3 pregnant women of chromosomal polymorphism,2 pregnant women of autosome translocation,1 pregnant woman of sex chromosome abnormality and 1 pregnant woman of chromosome inversion.Conclusion:Transabdominal cordocentesis guided by B type ultrasound has high success rate in examination of fetal chromosomal karyotype,which can satisfy the request of pregnant women with different indications of prenatal diagnosis,especially for those in the second and third trimester pregnancy,it is a safe and effective method for prenatal diagnosis.
