改良泪囊鼻腔吻合联合明胶海绵填塞疗效观察

摘　　要：目的观察改良泪囊鼻腔吻合联合明胶海绵填塞术在治疗慢性泪囊炎中的临床效果。方法对126例慢性泪囊炎患者随机分为A、B两组,A组行传统泪囊鼻腔吻合和凡士林纱条填塞63例;B组行改良泪囊鼻腔吻合联合明胶海绵填塞63例。观察术中及术后效果。结果 B组较A组手术时间短,术后患者舒适度高,无需特殊护理。随访3个月～5年。泪道冲洗:A组,通畅48例,狭窄7例,阻塞8例,治愈率87.3%;B组,通畅60例,狭窄2例,阻塞1例,治愈率98.4%。两组差异均有统计学意义(P<0.05)。结论改良泪囊鼻腔吻合联合明胶海绵填塞术具有操作简便、手术时间短、出血少、成功率高、术中及术后并发症少等特点,是治疗慢性泪囊炎的理想方法 。Objective A careful observation of clinical effects in the perfection of anastomosis of lachrymal sac and nasal cavity with gelatin sponge in the treatment of chronic lachrymal sacitis. Methods The division of 126 such chronic patients into A and B groups. Group A （63 patients） undergoing traditional anastomosis of lachrymal sac and nasal cavity with vaseline gauze, while Group B （63 patients） undergoing anastomosis of lachrymal sac and nasal cavity with gelatin sponge, in an attempt to make clinical observations of post - operation effects. Results Group B cost less time than Group A in operation, the patients in Group B feel better, without anyone to take care of them. The following visits have lasted from a season to five years, showing that after lachrymal - passage rinsing, there are 48 fee - move-ment cases, 7 stricture cases, 8 block cases, at the cure rate of 87.3% in Group A. In the meanwhile, there are 60 fee -movement cases, 2 stricture case, with only one block case, at the cure rate of 98.4%. There is a statistcs meaning in the difference of the two groups （P 〈 0.05 ）. Conclusion The perfection of anastomosis of lachrymal sac and nasal cavity with gelatin sponge is easy and convenient in operation, costing less operation time and hemorrhage, few in - operation and post - operation complications, at a higher rate of success, thus becoming a desired treatment of chronic lachrymal sacitis.
