穴位注射为主的综合疗法治疗后遗症期周围性面瘫临床研究

摘　　要：目的观察穴位注射结合综合疗法对后遗症期周围性面瘫的疗效。方法将150例患者以最小化随机方法分为治疗Ⅰ组(恩再适穴位注射+综合疗法)、治疗Ⅱ组(甲钴胺穴位注射+综合疗法)、对照组(综合疗法)各50例,于治疗前后分别观察并比较3组House Brackmann(H-B)面神经功能分级、面神经功能积分及面部肌电图改善情况。结果治疗Ⅰ组和治疗Ⅱ组H-B面神经功能愈显率显著高于对照组,面神经功能积分显著低于对照组,治疗Ⅰ组额肌、上唇方肌和下唇方肌的异常干扰相或混合干扰相募集电位改善率显著优于对照组(P<0.0167)。结论恩再适穴位注射结合综合疗法治疗后遗症期面瘫疗效优于甲钴胺,值得临床推广。Objective To investigate therapeutic effects of acupoint injection of Enzaishi, an extract from rabbit skin inflamed by vaccinia virus for injection, combined with comprehensive therapies in the treat- ment of sequelae of peripheral facial paralysis. Methods One hundred and fifty patients with sequelae of peripheral facial paralysis were averagely randomized into treatment group I （treated with acupoint injection of Enzaishi combined with comprehensive therapies）, treatment group Ⅱ （treated with acupoint injection of meeobalamine combined with comprehensive therapies） and control group （treated with comprehensive therapies alone）. Before and after treatment, House-Brackmann （H -B） facial nerve function ranking, facial nerve function scoring, and the improvement rate of abnormal electromyogram of facial muscles were observed and compared among the three groups. Results The rate of recovery and obvious effects on H- B facial nerve function ranking in treatment group Ⅰ was significantly higher than that in the control group, while the facial nerve function scores were significantly lower than those in the control group. The improvement rates of abnormal interference phase or mix interference phase recruitment potentials of fron- tal muscle, quadrate muscle of upper lip, and quadrate muscle of lower lip in treatment group I were significantly higher than those in the control group （P〈0. 016 7）. Conclusion Effects of acupoint injection of Enzaishi are superior to those of mecobalamine, combining with comprehensive therapies, in treating sequelae of peripheral facial paralysis, and it is worth spreading in clinical practice.
