婴幼儿持续性腹泻体锌水平变化及甘草锌佐治效果观察

摘　　要：目的观察婴幼儿持续性腹泻(Persistent diarrhea,PD)发锌及血锌水平变化;探讨甘草锌佐治婴幼儿PD的有效性。方法测定87例PD婴幼儿和100例健康婴幼儿的发锌、血锌水平,比较其差异及相关性;将87例PD婴幼儿随机分为两组,对照组常规治疗,补锌组在常规治疗基础上加用甘草锌治疗,并观察疗效。结果 PD婴幼儿的发锌、血锌水平低于正常婴幼儿(P<0.01),发锌与血锌高度相关(r=0.824,P<0.001);补锌组治疗2周后发锌、血锌值含量较治疗前增加(P<0.05)。补锌组患儿治疗总有效率(86.3%)明显高于对照组(76.2%),差别有统计学意义(P<0.05)。止泻时间补锌组(4.8±2.1)d,短于对照组(6.4±2.9)d,差别有统计学意义(P<0.05)。结论持续性腹泻婴幼儿发锌、血锌水平均低于健康儿;发锌检测可以较准确反映出锌营养水平;在常规治疗的基础上辅以甘草锌治疗PD可以缩短腹泻时间,提高临床治愈率,有明显的疗效。Objective To explore the change of body zinc level of infants with persistent diarrhea （PD） and efficacy of licorzinc supplementation on them. Methods 87 PD and 100 healthy infants were enrolled in this study, and the hair and blood zinc levels of them were detected and analyzed. The infants with PD were randomly divided into zinc treatment group and conventional therapy group. The patients in conventional therapy group were received conventional therapy for diarrhea; and patients in zinc treatment group took licorzinc supplement for successive 14 days while taking conventional therapy as the control. The different efficacy of the two groups and the changes of body zinc in treatment group were analyzed. Results Compared with normal infants, the ones suffering from PD had lower hair and serum zinc levels （P〈0.01）, the level of zinc in hair and serum had a significant correlation （r=0.824, P〈0.001）. The total efficacy of licorzinc treatment group （86.3%）, was much better than that of conventional therapy group 476.2%） （P〈0.05）. Compared with the conventional therapy group, the licorzinc treatment group had faster curative effects in number of stool times （P〈0.05）; After licorzinc treatment, the concentration of zinc hair and blood was higher than that before zinc supplement （P〈0.05）. Conclusion The infants suffering from PD have lower body zinc level. The level of zinc in hair can reflect the nutritional status of zinc. In addition to diarrhea conventional therapy, giving licorzinc treatment has better effect on enhancing curative effect of PD.
