二维和彩色多普勒超声检测眼球内常见恶性肿瘤

摘　　要：目的：研究视网膜母细胞瘤和脉络膜黑色素瘤的二维超声（2DUS）和彩色多普勒血流显像特征（CDFI）．方法：二维超声检查视网膜母细胞瘤或脉络膜黑色素瘤共33例，其中15例进行CDFI检查，全部病例均经手术（22例）或CT检查证实．结果：2DUS表现视网膜母细胞瘤以不规则形、类圆形和半圆形多见占95．4％（21／22例），钙化斑发生率较高77．3％（17／22例）．脉络膜黑色素瘤常见为蘑菇形，“挖空”征并伴视网膜剥离．CDFI证实视网膜母细胞瘤由视网膜中央动脉供血，而脉络膜黑色素瘤由睫状循环供血．结论：2DUS和CDFI检查相结合，能提高超声诊断及鉴别视网膜母细胞瘤和脉络膜黑色素瘤的正确性．AIM: To study two-dimensional ultrasonography (2DUS) and color Doppler flow imaging (CDFI) characteristics of ocular malignant neoplasm. METHODS: Thirty-three patients with retinoblastoma or melanoma of choroid were studied by 2DUS with fifteen of them examined by CDFI.The diagnosis of all cases were comfirmed by operation or CT. RESULTS: The ultrasonic indicates that most of the retinoblastoma is irregular,circle or semicircle (95. 4 % ), and the rate of calcification echo is high. The commonly encountered melanoma of choroid seems like mushroom and has 'cavern' sign with retinal detachment. It is demonstrated by CDFI that the blood feeding of retinoblastoma is central retinal artery while that of melanoma is ciliarys circulation.CONCLUSION: The combination of 2DUS and CDFI may obviously improve the accuracy of diagnosis and differentiation of retinoblastoma and melanoma of choroid.
