摘 要:目的:通过纳米炭示踪兔胃癌前哨淋巴结的对照研究,为纳米炭应用于胃癌的临床研究打下基础。方法:兔胃癌病灶周围浆膜下分别注射99Tcm-SC和纳米炭混悬液(卡纳琳),记录两种示踪法检测到前哨淋巴结(SLN)的例数、SLN数目及分布情况,计算两种示踪法检测SLN的敏感性、特异性、假阴性率和预测区域淋巴结转移的准确性以及SLN和非前哨淋巴结(NSLN)的转移率。结果:纳米炭与99Tcm-SC SLN的检出率及数目,显示SLN分布情况、SLN的转移率差异均无统计学意义(P>0.05)。两种示踪法检测SLN的敏感性、特异性、假阴性率、准确性差异均无统计学意义(P>0.05)。结论:纳米炭示踪胃癌前哨淋巴结效果与放射性核素99Tcm-SC是一致的,但纳米碳示踪法具有显影快而持久、无放射性、使用方便等优点,可替代放射性核素用于胃癌的临床研究。Objective: To evaluate the clinical application of nano-carbon in visualization of sentinel lymph node(SLN) in gastric cancer surgery.Methods: Rabbit gastric cancer model was established by transplantation of VX2 cells into the gastric serosa.Nano-carbon and ^99Tc^m-SC were injected respectively into the gastric wall in rabbit model of gastric cancer.The cases,the numbers and the distribution of SLN were documented.Then the sensibility,specificity,false negative rate and the accurate rate of local lymph node metastasis were calculated,the metastasis ratio of SLN,NSLN were evaluated.Results:As far as the isolating rate,the numbers,the distribution and the metastasis rate of SLN was concerned,no significant difference was shown between the two groups(P〉0.05).Moreover,the sensibility,specificity,false negative rate and the accurate rate of local lymph node metastasis were not difference between two groups(P〉0.05).Conclusions:Nano-carbon has a similar effect with ^99Tc^m-SC in SLN visualization.Furthermore,it can be used in gastric cancer surgery to replace radionuclide,considering its quick-detection,non-radioactivity and convenience.
