高强度聚焦超声治疗中晚期胰腺癌的初步研究

摘　　要：目的探讨HIFU治疗胰腺癌的有效性和临床可行性。方法 32例胰腺癌患者分别接受HIFU完全覆盖(7例)和部分覆盖(25例)治疗。通过主要脏器功能测定,超声检查,病灶体积变化,血清CA199、C3、C4、T淋巴细胞活性等指标观察HIFU的治疗效果。结果治疗后患者临床症状减轻,肿瘤组织超声回声增强,血供减少或消失,病灶缩小,血清CA199水平下降,血清C3、C4含量及T淋巴细胞活性上升。局部无明显并发症出现。结论 HIFU治疗胰腺癌是有效和可行的,该技术的应用将为临床非侵入性治疗胰腺癌提供一个新的局部治疗手段。Objective To evaluate the efficacy and clinical feasibility of high-intensity focused ultrasound in the treatment of pancreatic carcinoma. Methods Of 32 patients with pancreatic carcinoma, 7 cases had their tumor tissue completely destroyed by HIFU and 25 cases had their tumor tissues partly sonicated. Evaluation of efficacy was made on the basis of main organs functional tests, variations in sonography, tumor size, and serum CA199, C3, C4, T lymphocyte activity. Results Symptom alleviation, tumor shrinkage, enhance of sonography echo for tumor and reduction of boold supply for tumor, serum CA199 decrease, serum C3, C4, T lymphocyte activity increase were observed in patients after HIFU sonication. No local complication was seen. Conclusions The use of HIFU in the treatment of pancreatic carcinoma is effective and feasible. This new technique will offer a considerable noninvasivc therapy for local treatment of pancreatic carcinoma.
