长春瑞滨联合顺铂治疗晚期乳腺癌

摘　　要：目的:观察长春瑞滨(NVB)联合顺铂(DDP)治疗晚期转移性乳腺癌的疗效及不良反应。方法:41例乳腺癌术后复发转移者,既往接受过术后辅助化疗,化疗采用NVB25mg/m2,d1,8,DDP60-80mg/m2,分3天输注d2-4,每3周为1周期,至少用4个周期。结果:41例患者均可评价疗效,有效率(RR)为53.7%(22/41),疾病控制率(DCR)为73.2%(30/41),其中CR2例,PR20例,SD8例,PD11例,TTP8.6个月,OS24个月。主要不良反应为骨髓抑制和消化道反应,Ⅲ-Ⅳ度白血胞下降发生率为26.8%,无血小板下降者,Ⅲ-Ⅳ度恶心、呕吐发生率为14.6%,无肾功能损伤者,肝功Ⅱ度异常者9.8%,无Ⅲ-Ⅳ度者。结论:NVB联合DDP治疗晚期转移性乳腺癌有较好疗效,且对蒽环类和/或紫杉类治疗失败的患者亦有较好疗效,不良反应轻,耐受性好,可作为治疗复发转移性乳腺癌的解救方案。Objective：To observe the efficacy and adverse reaction of vinorelbine and cisplatin in treatment of advanced metastatic breast cancer. Methods： Forty- one patients with advanced metastatic breast cancer, treated with chemotherapy previously, were treated by NVB 25 mg/（ m2 · d）, d1.8, DDP 60 - 80 mg/m2, devided into 3 days, d2 _ 4, 3 weeks was one cycle,treated at least 4 cycles. Results ： Forty - one patients could be all evaluated, RR was 53.7% （22/41）, DCR was 73.2% （30/41） ,2 patients were CR,20 patients were PR,8 patients were SD,TTP was 8.6 months, OS was 24 months. The main adverse reactions were bone marrow suppression and gastrointestinal reactions. The rate of decrease of WBC was 26.8% （ III - IV ） , decrease of platelet was 0, hating and vomiting was 14.6% （ III - IV）, renal toxicity was 0,abnormal liver function was 9.8% （ II ）, abnormal liver function was 0（ III - IV）. Conclusion： The effect of chemotherapy with vinorelbine and cisplatin is good to treat advanced metastatic breast cancer and patients having been treated with anthracycline and / or taxane, and the side effect can be tolerated. So the chemotherapy with vinorelbine and cisplatin can be used as a rescued program in the treatment of recurrent and metastatic breast cancer.
