电刺激小脑治疗脑卒中后运动性失语的初步研究

摘　　要：目的评价电刺激小脑对脑卒中后运动性失语患者的治疗作用,探讨其治疗机制。方法选择在本院神经内科住院脑卒中运动性失语的患者66例,分常规药物组和药物加用脑循环功能治疗仪进行治疗的综合治疗组。疗程30天。均在治疗前后采用波士顿诊断性失语严重程度分级标准进行失语症严重程度分级及疗效评定。结果治疗前综合治疗组与常规药物组失语商(AQ)数值无显著差异。治疗后两组的AQ值均较治疗前明显增加,且综合治疗组AQ值(72.5±24.1)较常规药物组AQ值(59.7±25.5)增加更为显著(P<0.05)。治疗后综合治疗组患者的总有效率为81.0%,较常规药物组(55.1%)明显增高(P<0.05)。结论电刺激小脑与药物合用能显著改善脑卒中后运动性失语患者的语言功能,其机制可能与电刺激小脑激活小脑-大脑相关通路,调控语言的神经网络促进语言功能的恢复有关。Objective To explore the efficacy of electrical stimulation of cerebellum on the rehabilitation treatment of patients with post-stroke Broca aphasia.Methods Sixty-six patients suffering from post-stroke Broca aphasia were divided randomly into two groups to receive routine drugs treatment（drugs treatment group）or routine drugs plus electrical stimulation（complex therapy group）.The western aphasia battery was used to assess the patient＇s aphasia quotient（AQ）,and Boston Diagnostic Aphasia Examination was also conducted to assess the severity of aphasia.Duration of treatment was 30 days.Results There was no significant difference in AQ scores between two groups before treatment.AQ scores were improved significantly in both groups after therapy.Compared with drugs treatment group（59.7±25.5）,AQ scores was much better in complex therapy group（72.5±24.1）（P0.05）.Total effective rate of complex therapy group（81.0%）was obviously higher than those of drugs treatment group after treatment（55.1%）（P0.05）.Conclusion Electrical stimulation cerebellum combined with routine drugs treatment could significantly improve language function in the patients with post-stroke Broca aphasia,the mechanisms may presume that electrical stimulation cerebellum could active cerebellum-cerebrum pathway,this pathway could regulate language on neural network to promote patients＇ language function repair.
