腹腔镜直肠癌Miles手术与传统手术的临床对比研究

摘　　要：目的:探讨腹腔镜直肠癌Miles手术的安全性、有效性与可行性。方法:回顾分析47例低位直肠癌患者的临床资料,分别行腹腔镜Miles手术和常规Miles手术,比较两组患者的住院时间、手术时间、出血量、手术并发症、清扫淋巴结总数、术后切口愈合情况、住院费用、转移率等。结果:腹腔镜组与开腹组患者住院时间分别为(6.2±2.4)d、(9.8±2.1)d(P<0.05);手术时间分别为(1.8±0.4)h、(1.9±0.5)h(P>0.05);出血量为(106±6)ml、(210±16)ml(P<0.05);手术并发症发生率12%、30%(P<0.05);清扫淋巴结总数分别为(15.6±2.3)枚、(17.1±1.6)枚(P>0.05);住院费用(6 912±128)元、(7 261±536)元(P>0.05);转移率8%、10%(P>0.05)。结论:腹腔镜直肠癌Miles手术安全、有效、可行,与传统手术相比优势明显。Objective：To investigate the safety,efficacy and feasibility of laparoscopic Miles operation for rectal cancer.Methods：Laparoscopic Miles operation and open surgery were performed for 47 patients with low rectal cancer,the clinical data of which were collected and analyzed retrospectively,including the hospital stay,operative time,blood loss,complications,cleared lymph nodes,postoperative incision healing,hospitalization cost and metastasis rate.Results：The hospital stay of laparoscopic and open group was（6.2±2.4）d and（9.8±2.1）d,respectively（P〈0.05）.The operative time was（1.8±0.4）h and（1.9±0.5）h,respectively（P〉0.05）.The blood loss was（106±6）ml and（210±16）ml（P0.05）.The operative complication rate was 12% and 30%（P〈0.05）.The number of cleared lymph nodes was（15.6±2.3） and（17.1±1.6）,respectively（P〉0.05）;The hospitalization cost was（6 912±128）yuan and（7 261±536）yuan（P〉0.05）.The metastasis rate was 8% and 10%（P〉0.05）.Conclusions：Laparoscopic Miles operation for rectal cancer is safer,more effective and more feasible than open surgery.
