摘 要:目的研究膀胱癌患者尿脱落细胞中细胞角蛋白20(CK20)的表达探讨及其对膀胱癌的诊断意义。方法利用逆转录聚合酶链反应(RT-PCR)检测47例膀胱癌患者,19例非肿瘤患者及9例健康志愿者尿脱落细胞CK20mRNA的表达情况。结果 47例膀胱癌患者中,尿脱落细胞CK20阳性表达41例,阳性率87.2%;对照组28例中,阳性表达2例,阳性率7.1%。RT-PCR方法检测膀胱癌尿脱落细胞中CK20敏感性为87.2%,特异性为92.9%。PTa~T1期及PT2~T4期膀胱癌,其阳性率分别为85.7%(18/21)和88.5%(23/26),两者差异无统计学意义(P>0.05)。在G1、G2及G3级膀胱癌,其阳性率分别为85.7%(12/14)、85.7%(18/21)及91.7%(11/12),各级间差异无统计学意义(P>0.05)。CK20的表达同膀胱癌的分期、分级无关。结论 CK20是一种较为理想的膀胱肿瘤标记物,利用RT-PCR检测尿脱落细胞中CK20可用于膀胱癌的诊断。Objective To investigate the expression of cytokeratin 20 in exfoliated urothelial cells of patients with bladder cancer and to evaluate its diagnostic significance. Methods The reverse transeriptasepolymerase chain reaction(RT-PCR) was used to determine the expression of CK20 in exfoliated urothelial cells of 47 patients with bladder cancer, 19 patients with non-malignant urologic disease and 9 healthy subjects. Results The positive expression of CK20 was found in 41 patients with bladder cancer, however,, only 2 positive cases were detected in control group. The sensitivity and specificity of the CK20 mRNA assay for bladder cancer was 87.2% and 92.9% respectively. The positive rate of CK20 in PTa - T1 and PT2 - T4 patients was 85.7% ( 18/21 ) and 88.5% (23/26) respectively. The positive rate of CK20 in G1, G2 and G3 patients was 85.7% ( 12/14 ), 85.7% ( 18/21 ) and 91.7% (11/12) , respectively. The CK20 expression in exfoliated urothelial cells was not related to the tumor grades and stages. Conclusion CK20 may serve as a appropriate tumor marker for bladder cancer. The detection of CK20 mRNA in exfoliated urothelial cells by RT-PCR can be used in the diagnosis of bladder cancer.
