具有中央颞区棘波的良性儿童癫痫46例分析

摘　　要：目的总结具有中央-颞区棘波放电的儿童良性癫痫(BECCT)的临床特点及脑电图特征.方法对46例BECCT的患者进行分析,包括家族史、热性惊厥史、临床表现、脑电图变化及治疗效果预后情况.结果起病年龄3.2～12.8岁,35例患儿在5～10岁(35/46,76.09%).有家族史8例(17.4%),热性惊厥史6例(13.04%),发作和睡眠密切相关者37例(37/46,80.43%).部分性发作36例(36/46,78.26%),强直-阵挛发作6例(6/4613.04%).发作间期脑电图可见一侧或双侧中央和/或中颞区有尖波或棘波发放.清醒脑电图阳性率25例(54.35%),睡眠脑电图阳性率42例(91.3%).绝大多数患儿应用抗癫痫药物(AEDs)治疗疗效满意,占43例(93.45%).结论 BECCT早期准确的诊断有助于治疗和判断预后,从而减轻患儿及家庭的负担.Objective To summarize the clinical and EEG characteristics of benign epilepsy with centro-temporal spikes（BECCT） in children.Methods The clinical and EEG data of 46 children with BECCT were retrospectively analyzed. The data included family history,febrile seizure（FS） ,clinical manifestations,EEG characteristics and treatment effect.Results The onset age of this disease was 3.2～12.8 years,and 35 patients aged 5 to 10 years（35/46,76.09%） .8 patients（17.4%） were associated with family history and 6 cases（13.04%） were with febrile seizure history.The seizures of 37 cases（37/46,80.43%） were associated with sleep.36 cases（36/46,78.26%） were partial seizures,and 6 cases（6/46,13.04%） were generalized seizures.EEG demonstrated that there were sharp or spike waves of interior discharge in the centro-temporal urea of one or two sides,25（54.35%） cases were positive in awake EEG,and 42（91.3%） cases were positive in sleep EEG.43（93.45%） cases were responsive to anti-epileptic drugs.Conclusion The characteristics of BECCT are as follows：partial epilepsy related to sleep,partial seizure types and sharp or spike waves of EEG in centro-temporal area.EEG sleep induction test could increase positive diagnostic rate of this disease,and the prognosis of this disease was fine.The early diagnosis is helpful for the patients＇ prognosis,and can alleviate psychological suffering of the patients and their families.
