胸大肌皮瓣修复口腔颌面部巨大组织缺损的临床应用

摘　　要：目的：探讨胸大肌皮瓣修复口腔颌面部大范围软组织组织缺损的方法、临床经验及疗效。方法：1996年5月～2008年11月，对24例各种原因所致的口腔颌面部大型缺损，采用胸大肌皮瓣予以修复。结果：23例胸大肌皮瓣全部成活，均获得较满意的外观及功能。1例发生皮瓣部分坏死。术后1a随访患者16人，颌面形态良好。结论：带蒂胸大肌皮瓣血供可靠，组织量多，能够修复口腔颌面部较大面积缺损，且修复的灵活性高，安全可靠，成功率高，适用于各种原因所致的口腔颌面部巨大组织缺损的一期修复和重建。Objective：To review the method of pectoralis major myocutaneous flaps in reconstructing large defects in the oral and maxillofacial region and its clinical experience and effect. Methods：Twenty four cases with large defects in the oral and maxillofacial region caused by different reasons were reconstructed with pectoralis major myocutaneous flaps from May, 1996 to November,2008. Results：Twenty three flaps survived and achieved satisfactory function and appearance. Only one flap had partial necrosis. 16 patients who were followed up for 12 months were satisfied with the appearance of the maxillofacial region. Conclusions： Pectoralis major myocutaneous flap with adequate vascularization and enough tissue, can be used to reconstruct various large defects in the oral and maxillofacial region. Furthermore, this technique has high success rate and is reliable and versatile in the reconstruction.
