摘 要:目的探讨十堰地区原发性肝癌与幽门螺杆菌感染之间的关系。方法 80例原发性肝癌组织标本和10例正常肝组织标本,分别采用螺杆菌培养、PCR扩增细菌16SrDNA通用序列以及幽门螺杆菌特异基因(26kD基因)和相关功能基因(cagA、vacA、rps4、glmM)、免疫组化法检测各组织标本中有无幽门螺杆菌感染。结果①未能从组织标本中培养出螺杆菌。②80例癌组织标本中有50例检出16S rDNA基因,阳性率为62.5%;正常肝组织未检出;50例16S rDNA基因阳性标本中有8例cagA基因阳性(16%),37例26kD基因阳性(74%),9例glmM基因阳性(18%),未能扩增出vacA基因以及rps4基因。③80例肝癌组织标本中26例检出幽门螺杆菌,阳性率为32.5%,正常肝组织中未检出。结论十堰地区原发性肝癌组织内存在幽门螺杆菌基因物质,且感染率较高,幽门螺杆菌的感染与原发性肝癌的发生存在相关性。Objective To investigate the relationship between primary hepatocellular carcinoma(HCC)and Helicobacter pylori infection in Shiyan area. Methods Liver samples from 80 patients with primary hepatocellular carcinoma and 10 normal liver tissues as controls were studied.Liver tissue samples were firstly submitted to microaerobic culture.Then,a 16SrDNA-based sequence was amplified by polymerase chain reaction(PCR),and a further developed search for Helicobacter pylori(26 kD gene,cagA,vacA,rps4,glmM)was also carried out by PCR.The immunohistochemical staining for Helicobacter pylori was performed in liver samples. Results ①The Helicobacter pylori was not successfully cultured in liver tissue samples.②Of 80 samples of primary hepatocellular carcinoma,50(62.5%)were positive for Helicobacter-specific 16SrDNA gene,while no one was positive in controls(P0.01).Of these 50 positive samples,37(74%)were positive for the 26 kD protein primers of Helicobacter pylori,8(16%)for the cagA,9(18%) for the glmM,and no one for vacA and rps4 genes.③Helicobacter pylori was found in 26 of 80 hepatocellular carcinoma samples(32.5%),but not in controls(P0.01). Conclusion DNA of Helicobacter pylori exist in primary hepatocellular carcinoma tissues,and Helicobacter pylori infection may exist in the liver tissues of patients with primary hepatocellular carcinoma with a high infection rate,suggesting an association between Helicobacter pylori infection and primary hepatocellular carcinoma.
