摘 要:目的:确定广西壮汉族正常足月新生儿生后1~7天血清胆红素的动态变化值,为临床诊断奠定基础。方法:①用微量法测定广西地区正常足月新生儿生后第1~7天血清胆红素,分别记录其测定值。②应用生化法与微量法相结合方法,即用静脉生化法与微量法同步测定72例新生儿血清胆红素,用微量法测定值代入生化法与微量法测定值建立的直线回归方程,获取胆红素校正值。结果:①正常足月新生儿1~7天血清胆红素分别为(72.17±14.39)μmol/L、(128.93±28.92)μmol/L、(174.69±33.07)μmol/L、(186.46±34.23)μmol/L、(174.25±37.60)μmol/L、(154.49±42.76)μmol/L、(141.41±44.97)μmol/L,平均峰值(186.46±34.23)μmol/L。②壮族新生儿血清胆红素平均峰值比汉族略高,壮汉两族1~7天血清胆红素的比较差异无统计学意义(P>0.05)。③微量法与静脉生化法胆红素测定值有高度的相关性,两法测定值比较差异无统计学意义(P>0.05);校正后的胆红素值与静脉生化法测定值比较,差异无统计学意义(P>0.05)。结论:广西地区壮汉族足月新生儿生理性黄疸生后1~7天血清胆红素动态变化值,为临床对新生儿黄疸的合理干预提供科学依据。生化法与微量法相结合的方法测定血清胆红素准确、便捷,是一种动态监测新生儿血清胆红素的理想方法。Objective: To determine the dynamic changes of serum bilirubin levels in normal full-term neonates of Zhuang nation- ality and Han nationality 1 ~ 7 days after birth,provide a basis for clinical diagnosis. Methods: The serum bilirubin levels of normal full-term neonates in Guangxi 1 ~ 7 days after birth were detected by micromethod and recorded. The serum bilirubin levels of 72 neonates were detected by venous biochemical method and micromethod at the same time,then the results of micromethod were substituted into the linear regression equation established by results of venous biochemical method and micromethod,adjusted value of bilirubin was obtained. Results: The serum bilirubin levels of normal full-term neonates in Guangxi 1 ~ 7 days after birth were ( 72. 7 ± 14. 39) μmol/L,( 128. 93 ± 28. 92) μmol/L,( 174. 69 ± 33. 07) μmol/L,( 186. 46 ± 34. 23) μmol/L,( 174. 25 ± 37. 60) μmol/L,( 154. 49 ± 42. 76) μmol/L and ( 141. 41 ±44. 97) μmol/L,respectively,the peak value was ( 186. 46 ±34. 23) μmol/L on average. The peak value of bilirubin on average in neonates of Zhuang nationality was slightly higher than that in neonates of Han nationality,there was no significant difference in serum bili- rubin levels 1 ~ 7 days after birth between neonates of Zhuang nationality and neonates of Han nationality ( P ﹥ 0. 05) ; there was a high correlation of bilirubin level between micromethod and venous biochemical method,but there was no significant difference in serum bilirubin level between micromethod and venous biochemical method ( P ﹥ 0. 05) ; there was no significant difference between adjusted value of bilirubin and serum bilirubin level detected by venous biochemical method ( P ﹥ 0. 05) . Conclusion: The dynamic changes of serum bilirubin level in normal full-term neonates of Zhuang nationality and Han nationality 1 ~ 7 days after birth provide a scientific basis for reasonable intervention of neonatal jaundice,joint detection of venous
