手术治疗70岁以上高龄食管癌、贲门癌75例临床研究

摘　　要：目的总结70岁以上高龄食管癌、贲门癌手术治疗经验。方法回顾性分析梅州市人民医院手术治疗的70岁以上高龄食管癌、贲门癌75例临床资料。结果 75例中术前伴发其他疾病共51例,占68%。手术切除69例,切除率92%。术后发生并发症26例,占34.7%,其中胸积液10例;肺炎8例;心房纤颤2例;呼吸衰竭2例;吻合漏4例;死亡1例。随访1～7年,1年生存率73.3%,3年生存率38.7%,5年生存率22.6%。结论 70岁以上高龄食管癌、贲门癌可耐受手术治疗,此类患者伴发病较多,加强围手术期处理可提高手术安全性。Objective To summarize the experience of the operations in treating 75 above-70-year patients with esophageal or cardiac cancer.Methods The clinical data of the operations treating 75 above-70-year patients with esophageal or cardiac cancer were retrospectively analyzed.Results In 75 patients,51 cases（68%）were complicated with other diseases before operation.69 cases（92%）were treated by surgical removal of tumor.26 cases（34.7%）had complications as following：with pleural effusion（10 cases）,with pneumonia（8 cases）,with atrial fibrillation（2 cases）,respiratory failure（2 cases）,pleural effusion（4 cases）,which 1 case died.Via 1 to 7-year follow-up,1-year survival rate,3-year survival rate,and 5-year survival rate were 73.3%,38.7% and 22.6% respectively.Conclusion The above-70-year patients with esophageal or cardiac cancer can tolerate operation,but they had more complications.The sufficient preparation in perioperative period can improve the surgery safety.
