直肠癌术中骶前静脉丛大出血的原因及处理

摘　　要：目的总结直肠癌根治术中骶前静脉丛大出血的原因和处理方法。方法对1980年1月至2008年12月我院直肠癌根治手术中发生的骶前静脉丛大出血5例病例,术中出血量>600ml2例,600～1000ml2例,1500ml1例。全部采用纱布压迫止血法止血。结果止血后未再发生出血,5例患者均痊愈出院。结论肿瘤的浸润及手术操作不当是引起骶前静脉丛大出血的主要原因。直肠癌根治手术中按骶骨解剖层次正确操作是预防术中骶前静脉丛大出血的重要因素,纱布压迫止血是处理骶前静脉丛大出血简便有效的方法。Objective To summarize the reason and the methods for preventing and treating the presavenous hemorrhage in the surgery of rectum cancer, Methods The patients underwent the radical surgery of presacral renons hemorrhage of rectum cancer from January 1980 to December 2008 were analyzed retrospectively. 5 patients with presacal venous hemorrhage introperatively, and 5 patients all underuwent Miles opera- tion. 2 patients of the introperative blood loss was under 600 ml,600 - 1000 ml in 2patients, 1500 ml in 1 patients, on the basis of the bleeding site conditious with carbasus were adopted to control the bleeding. Results No further bleeding occurred, fiver fully recovered from the operation. Conclusions The main reason for presa- cral venous hemorrhage is toumor infiltration and incorrect procedure. Dissecting the presacral structure careful- ly is the key pint of preventing presacral venous hemorrhage in the surgery of rectum cancer. Packing with carbasus is an effective method to treat the presactal venous hemorrhage.
