基托式和非基托式上颌快速扩大器对牙弓的影响

摘　　要：目的:对比带塑料基托和无基托的上颌快速扩大器(RME)扩弓的临床效果。方法:对临床上使用上颌快速扩大器进行治疗的28例患者的上颌牙弓扩大效果进行分析。28例均为混合牙列期或恒牙列早期上颌牙弓狭窄或后牙反的患者,其中14例使用带塑料基托的上颌快速扩大器进行扩弓,另外14例则使用无塑料基托的上颌快速扩大器。扩弓前及拆除扩弓装置时分别取石膏模型并拍摄上颌前部咬合片,测量相关项目,进行统计学分析。结果:两组患者牙弓扩大程度的差异无统计学意义,但基托式组腭中缝扩开的效果更明显,两组间扩大腭中缝效果的差异有统计学意义。结论:带塑料基托的上颌快速扩大器和无基托的上颌快速扩大器均能有效地扩大牙弓,但前者对腭中缝的扩开效果好于后者。Objectives To compare the clinical results of rapid maxillary expansion（RME） appliances with and without split acrylic palate.Methods 28 patients treated with RME are compared to see the resulting expansion of their maxillary dental arches.These 28 patients are either in mixed dentition period or early permanent dentition period with either narrow maxillary arches or Posterior crossbites.14 of them are treated by RME with acrylic palate,while the others are treated by RME without acrylic palate.Plaster casts and maxillary anterior occlusal films are obtained respectively before and right after treatment.These plaster casts and films provide relevant data for statistical analysis.Results there are no statistically significant differences between these two groups in terms of the expansion of their maxillary dental arches.However,the group using acrylic palate showed more expansion in the midpalatine raphe,which is statistically significant.Conclusion RME with and without acrylic palate are equally effective in widening the maxillary dental arch width,while the former excel in expanding the midpalatine raphe.
