HPV-DNA和TCT检测在宫颈癌前病变筛查中的临床价值

摘　　要：目的应用基因芯片HPV-DNA和液基薄层细胞学(TCT)方法筛查宫颈癌前病变及早期宫颈癌。方法对妇科门诊1076例患者行HPV-DNA和TCT筛查。对高危HPV阳性并且TCT≥未明确诊断意义的不典型鳞状上皮细胞(ASCUS)或者虽HPV检测结果阴性,但TCT为低度鳞状上皮内瘤变(LSIL)及以上病变患者共224例,均经阴道镜下宫颈活组织检查或宫颈管诊刮,以病理结果作为最终诊断。结果 1076例患者中,高危HPV阳性检出率13.9%(150/1076),TCT检查者ASCUS阳性检出率15.2%(164/1076)。TCT和HPV均阳性者90例,8.4%(90/1076);2例宫颈浸润癌,CINⅠ～Ⅲ者为78例,诊断符合率占86.7%。结论 HPV-DNA联合TCT检测可提高宫颈癌前病变的诊断符合率。是高效、便捷的宫颈癌前病变的筛查方法 。Objective To study the diagnostic value of gene chip HPV-DNA and thin cytology（liquid）technique（TCT）in screening anterior cervical pathological changes and cervical cancer in early stage.Methods A total of 1076 patients received a dual screening with TCT and gene chip HPV-DNA.Of 1076 patients,224 cases were high risk HPV positives with low degree ASCUS or LSIL,who were examined with colposcopy and biopsy for pathological diagnosis.Results Of 1076 patients,the overall HPV rate was 13.9%（150/1076）,and ASCUS positive rate with TCT was 15.2%（164 of 1076）.Total positive rate with both techniques was 8.4%（90/1076）with an accordance rate of 86.7%.Conclusion Gene chip HPV-DNA plus TCT may improve the accordance rate in screening anterior cervical pathological changes and cervical cancer in early stage.
