摘 要:目的:探讨细胞角蛋白5(cytokeratin 5,CK5)、CK8和乳腺癌耐药蛋白(breast cancer resistance protein,BCRP)在乳腺上皮良、恶性增生病变中的表达及意义。方法:应用免疫组织化学法分别检测89例正常乳腺、乳腺增生、导管上皮非典型增生、导管原位癌和浸润性导管癌组织中CK5、CK8及BCRP的表达情况,分析CK5、CK8及BCRP的表达与乳腺病变组织分化程度之间的关系。结果:在正常乳腺、乳腺增生、导管上皮非典型增生、导管原位癌和浸润性导管癌组织中CK5和CK8的阳性表达率逐渐下降,BCRP的阳性表达率则逐渐上升。CK5和CK8在乳腺良性增生组织(乳腺增生和导管上皮非典型增生)中的阳性表达率明显高于乳腺癌(导管原位癌和浸润性导管癌)组织(P<0.01),BCRP在乳腺癌组织中的表达明显高于乳腺良性增生组织(P<0.01)。在乳腺浸润性导管癌中,CK5阳性表达主要集中在外层尚未受侵袭的基层细胞中,肿瘤细胞中没有表达或很少表达;CK8则主要在肿瘤细胞中表达,基层细胞中完全不表达;66.7%的浸润性导管癌患者呈CK5-/CK8+表型。结论:CK5、CK8和BCRP在不同乳腺病变组织中的表达不同,3者联合检测可能有助于乳腺病变的病理学诊断。Objective:To investigate the expressions of cytokeratin 5 (CK5), CK8 and breast cancer resistance protein (BCRP) in the benign and malignant lesions of human mammary gland and their significance.Methods:Eighty-nine samples of human mammary gland tissues were collected in this study. Immunohistochemical technique was used to detect the expressions of CK5, CK8 and BCRP. The correlation of their expressions with the differentiation of the mammary gland tissues with benign and malignant hyperplasia was analyzed. Results:The positive expressions of CK5and CK8 gradually decreased but the expression of BCRP increased step by step in normal mammary gland, hyperplasia, atypical hyperplasia, ductal carcinoma in situ, and infiltrating ductal carcinoma tissues. The expressions of CK5 and CK8 were markedly higher in benign hyperplasia tissues (hyperplasia plus atypical hyperplasia) than those in breast cancer tissues (ductal carcinoma in situ plus infiltrating ductal carcinoma) (P〈0.01). In contrast the expression of BCRP was significantly higher in breast cancer tissues than that in breast tissues with benign hyperplasia (P〈0.01). In infiltrating ductal carcinoma, CK5 was mainly expressed in basal lamina not infiltrated by tumor cells. There was no or little expression of CK5 in cancer cell. Contrary to CK5, CK8 was mainly expressed in cancer cells and was not expressed in basal lamina. There was 66.7% infiltrating ducal carcinoma displayed CK5^-/CK8^+ phenotype. Conclusion:The expression patterns of CK5, CK8 and BCRP were different in various breast tissues. Combined detection of the three proteins may be helpful for the pathological diagnosis of lesions of mammary gland.
