摘　　要：目的:探讨CD146基因的表达与尿路上皮癌的发生及浸润和转移之间的关系。方法:采用实时荧光定量PCR技术检测尿路上皮癌组织及其癌旁正常尿路上皮黏膜中CD146基因的表达情况,同时应用免疫组织化学法检测CD146蛋白的阳性表达率。结果:实时荧光定量PCR检测结果显示,CD146 mRNA在尿路上皮癌组织中的表达量明显高于正常尿路上皮黏膜(P<0.01),且随着肿瘤病理分级和临床T分期的升高以及淋巴结转移的出现,其表达水平有所升高,差异有统计学意义(P<0.01)。CD146蛋白在尿路上皮癌组织中的阳性表达率明显高于癌旁正常尿路上皮黏膜(P<0.01),且随着尿路上皮癌病理分级的升高,其阳性表达率也增加(P<0.01);CD146蛋白在浸润性癌中的阳性表达率明显高于浅表性癌(P<0.05);有淋巴结转移者的CD146蛋白阳性表达率明显高于无淋巴结转移者(P<0.05)。CD146蛋白与CD146 mRNA在尿路上皮癌中的表达水平之间存在相关性(P<0.05),表明CD146蛋白的表达增强与CD146 mRNA表达的上调相关。结论:CD146基因及其蛋白的表达上调与尿路上皮癌的病理分级、肿瘤浸润深度以及淋巴结转移相关,有望成为评判尿路上皮癌恶性程度、转移潜能及预后的一个新指标。Objective：To investigate the relationship between CD146 gene expression and the invasion and metastasis of urothe-lial cancer.Methods：The expressions of CD146 mRNA in urothelial cancer and adjacent normal urothelial tissues were detected by real-time fluorescent quantitative PCR（RFQ-PCR）. The CD146 protein was detected by immunohistochemistry （IHC） method in urothelial cancer tissues and their normal mucosa tissues.Results ： QRT-PCR showed that the average quantity of CD146 mRNA in urothelial cancer was much higher than that in normal urothelial tissues （P〈0.01）. The level of CD146 mRNA increased with the increase in the pathological grade and the clinical T stage,as well as the lymph node metastasis. The difference was significant （P〈0.01）. The expression of CD146 protein was significantly higher in tumor tissues than that in adjacent normal urothelial tissues （P〈0.01）. The expression of CD146 in invasive urothelial cancer was significantly higher than that in superficial cancer （P〈0.05）. And it was significantly higher in the group with lymph node metastasis than that without lymph node metastasis （P〈0.05）. The expressions of CD146 mRNA and the CD146 protein correlated with each other, indicating that the enhanced expression of CD146 protein was due to the up-regulation of CD146 mRNA. Conclusion：Up-regulation of CD146 gene and protein was related with pathological grade, depth of invasion and lymph node metastasis. Expression of CD146 may become a novel index for evaluating the malignant degree, metastatic potential and the prognosis of urothelial cancer.
