摘 要:目的:研究Notch-1在食管鳞癌及癌旁正常组织中的表达,并联系临床病理特征,探讨Notch-1在食管癌发生、发展中的作用及其临床意义。方法:收集并保存2008年4月至2008年9月四川大学华西医院胸外科手术切除的食管鳞癌及癌旁正常组织(距离肿瘤至少5cm之外的正常食管黏膜组织)标本,利用RT-PCR方法检测41例食管鳞癌及癌旁正常组织中Notch-1基因mRNA水平的相对表达强度,并分析其与肿瘤侵袭深度(T)、淋巴结转移(N)、TNM分期、分化程度等临床病理特征之间的关系。结果:所有标本中均可见到Notch-1的电泳条带,即均检测到Notch-1基因mRNA水平的表达。半定量分析显示食管鳞癌中Notch-1的相对表达强度为0.51±0.37,癌旁正常组织中为0.32±0.20,两者有显著性差异(t=3.52,P=0.001)。按照肿瘤侵袭深度(T)、淋巴结转移(N)、TNM分期分化程度等不同的临床病理特征划分亚组后,食管鳞癌与癌旁正常组织中的差异显著性仍然存在。而在食管鳞癌组中,各亚组之间的差异均无统计学意义。结论:Notch-1可能在食管鳞癌发生中起作用,而与食管鳞癌病变的进展可能关系不大。Objective: To explore the role of Notch-1 in the tumorigenesis and progression of human esophageal carcinoma and its clinical significance. Methods: Esophageal squamous cell carcinoma (ESCC) and adjacent normal tissue (the normal esophageal mucosa being at least 5 cm apart from the edge of the tumor) specimens were collected in our hospital between April 2008 and September 2008. A total of 41 cases were examined by RT-PCR to detect the relative expression intensity of Notch-1 gene at the mRNA level Also, the differences in Notch-1 relative expression intensity between tumor and normal tissue were analyzed. Moreover, its relationship to the clinicopathological features, such as depth of tumor inva- sion (T), lymph node metastasis (N), TNM stage, and degree of differentiation, was also assessed. Results: (1) Notch-1 electrophoretic bands could be seen in all specimens, indicating that Notch-1 gene was expressed in all specimens at the mRNA level (2) Semi-quantitative analysis showed that the relative expression intensity of Notch-1 in ESCC was 0.51 ± 0.37, while in adjacent normal tissue it was 0.32±0.20, with a significant difference (~3.52, P=0.001). (3) After stratification by the clinicopathologic features mentioned above, significant differences in Notch-1 expression between ESCC and adjacent normal tissue were found in all sub-groups. However, in ESCC, there was no significant difference among sub-groups. Conclusion: Notch-1 may play a role in ESCC tumorigenesis but may be unrelated to the progression of ESCC.
