白内障超声乳化术后角膜内皮细胞损伤相关因素分析

摘　　要：目的探讨造成白内障超声乳化手术后角膜内皮细胞损伤的相关因素。方法观察182例手术眼,分别记录手术医生、核硬度、超声总能量、术前角膜内皮细胞密度、黏弹剂类型、灌注液、人工晶状体类型。术后观察角膜内皮细胞变化,计算术后1周和3个月后细胞损失率,并进行多因素相关回归分析。结果角膜内皮细胞损失率在术后1周与手术医生、核硬度和灌注液类型密切相关(P<0.05);在术后3个月与手术医生、核硬度和术前角膜内皮细胞密度密切相关(P<0.05)。结论为了减少白内障手术对角膜内皮细胞的影响,有必要行角膜内皮细胞检查;对手术医生应提高手术技巧、手术中应稳定前房、减少能量和机械损失;有手术适应证的患者应尽早手术。Objective To investigate the cause of the corneal endothelial cells＂ injury after the phacoemulsification cataract surgery. Methods One hundred and eighty - two operated eyes were observed, and the surgeon, nuclear hardness, total energy of ultrasound, density of preoperative corneal endothelial cell, type of viscoelastic agent, perfusate, and type of intraocular lens were recorded . The postoperative corneal endothelial cells were observed, the cell loss rate was calculated in 1 week and 3 months, and the multivariate regression was analyzed with SPSS statistical software. Results The loss rate of corneal endothelial cells was closely related to the surgeon, nuclear hardness and type of perfusate in 1 week and to the surgeon, nuclear hardness and density of preoperative corneal endothelial cells in 3 months after surgery. Conclusion In order to reduce loss of the corneal endothelial cells during the cataract surgery, the endothelial cells need to be checked. The surgeon should improve surgical technique, stabilize anterior chamber, and reduce energy and mechanical loss to the endothelial cells. The patient who has operative indication should receive the surgery as soon as possible .
