EPOCH方案治疗T细胞非霍奇金淋巴瘤26例

摘　　要：目的观察EPOCH方案治疗T细胞非霍奇金淋巴瘤疗效和副作用。方法 26例T细胞非霍奇金淋巴瘤患者采用EPOCH方案治疗。EPOCH方案：依托泊苷每天50 mg/m2、多柔比星每天10 mg/m2、长春新碱每天0.4 mg/m2,持续静脉滴注24 h,第1~4天;环磷酰胺（CTX）750 mg/m2,静脉滴注,第6天;强的松每天60 mg/m2,口服,第1~6天;21天为1个周期,共进行4~6周期,中位周期数为5。结果 26例患者总有效率92.3%（23/26）,完全缓解（CR）率61.5%,部分缓解（PR）率30.8%,肿瘤进展时间（TTP）35.14±2.19个月,主要不良反应为骨髓抑制和脱发。结论 EPOCH方案治疗T细胞非霍奇金淋巴瘤疗效好,对B症状（发热、盗汗及消瘦）控制好,副作用小,值得临床进一步研究。Objective To evaluate the efficacy and adverse effects of EPOCH regimen for patients with T-cell non-Hodgkin＇s lymphoma. Metheds 26 patients were treated by EPOCH regimen：Etoposide 50mg/m2,Doxorubicin 10mg/m2,Vincristine 0.4mg/m2 infused over 96 h on day 1 to 4,Cyclophosphamide 750 mg/m2 iv on day 6,oral Prednisone 60 mg/m2 iv on day 1 to 6,repeated every 21 days.All patients received 4 to 6 cycles of chemotherapy,median cycle was 5.Result Among the 26 patients treated by EPOCH regimen,the reponse rate was 92.3%（24/26）,including 16 complete reponse（CR61.5%） and 8 partial reponse（CR30.8%）.The mean TTP was 35.14±2.19 months.Main adverse effect was myelosuppression and alopecia. Conclusions The EPOCH regimen was effective for patients with T-cell non-Hodgkins lymphoma,eliminated B symptoms and its adverse effects were mild.EPOCH regimen is warranted with clinical study.
