胰腺实性假乳头状瘤的CT诊断与鉴别诊断

摘　　要：目的:探讨胰腺实性假乳头状瘤的CT表现,提高对该病的诊断水平。方法:回顾性分析了5例经手术病理证实的胰腺实性假乳头状瘤患者的CT资料,均行CT平扫及增强。结果:5例患者中4例为年轻女性,男性1例,均无黄疸。位于胰头2例、胰尾2例,胰体尾均受累1例。CT平扫表现为圆形、类圆形低密度或混杂密度团块影,体积较大,增强后肿瘤实性部分及包膜可见逐渐强化,囊性部分无强化。4例病灶内见点状钙化。结论:胰腺实性假乳头状瘤影像学表现有一定特征性,结合临床资料可在术前做出诊断。Objective：To study the CT features of solid-pseudopapillary tumors of pancreas（SPTP） and to improve the diagnostic level.Methods：The CT materials of 5 cases with surgery and pathology proved SPTP were retrospectively analyzed.All of the 5 cases underwent CT scan without and with contrast enhancement.Results：4 cases were young women,1 case was a young man,all without jaundice.A large round or oval-shape cystic-solid mass was showed with low or heterogeneous attenuation on plain CT.2 masses located at the head of the pancreas, 2 located at the tail,the other located at both the body and the tail.After contrast administration,the solid component and the membrane of the mass showed enhancement while no enhancement was assessed in the cystic component.Conclusion：There are characteristic CT findings of SPTP,as long as the image findings in consistent with patient＇s clinical symptoms,the diagnosis of SPTP should be considered before surgery.
