电刺激小脑顶核对脑瘫患儿智力发育的影响

摘　　要：目的探讨小脑顶核电刺激治疗对脑瘫患儿智力发育的临床效果。方法将2007年2月至2009年2月来我院进行康复治疗的脑瘫患儿120例作为研究对象。患儿按就诊时间随机分为两组,治疗前对两组患儿进行格塞尔(Gesell)量表智力测试,取其中的大运动能力项和适应能力项进行评估,制定治疗方案。两组治疗均采用针灸、按摩、脑神经营养药、运动疗法、语言训练及特殊教育等综合康复措施,观察组同时应用电刺激小脑顶核进行治疗。治疗后采用Gesell量表中的大运动能力项、适应能力项进行评定。施测时选用与被试儿童年龄最近的分量表进行。据其表现测出儿童发展的成熟年龄,经转换得出发育商(DQ)作为最后的评定指标。结果治疗3个月后,两组患儿的DQ均有提高(P<0.05),观察组提高更为明显(P<0.01)。结论小脑顶核电刺激方法简便,安全可靠,是治疗脑瘫患儿的有效手段之一。小脑顶核电刺激对脑瘫患儿的智力发育有显著意义。Objective To determine the clinical effect of electric stimulation on cerebellar fastigial nucleus （FN） for intellectual development of children with cerebral palsy. Methods Altogether 120 children with cerebral palsy accepted rehabilitation services in this hospital from February 2007 to February 2009 were studied, they were randomly allocated into two groups according to visiting time, and the intelligence test of children had been examined by GDS before treatment. Treatment protocols were made accoding to estimation of gross motor and adaptive behavior by GDS. The evaluation of gross motor and adaptive behavior was adopted with GDS after treatment. Subscales were closely chosen based on children＇s age. The developmental age of children was measured and converted to development quotient （DQ） as ultimate evaluation indices. Results After treatment for 3 months, levels of DQ were raised in children of both groups（ P 〈 0.05 ）. The level of DQ was much higher in children of observation group than that of those in control group （ P 〈 0.01 ）. Conclusion Electric stimulation on cerebellar fastigial nucleus is one of efficient methods with its advantages of convenience, safety and reliability. Electric stimulation on eerebellas fastigial nucleus is effective for intellectual development in children with cerebral palsy.
