GP方案化疗前后加用平消片治疗中、晚期非小细胞肺癌临床观察

摘　　要：目的观察平消片合并GP方案(吉西他滨+顺铂)化疗及单用GP方案化疗治疗中、晚期非小细胞肺癌(NSCLC)患者的疗效及不良反应。方法选择80例接受化疗的中、晚期NSCLC患者随机分为单用化疗组(对照组)40例,化疗前后加用平消片组(治疗组)40例,治疗周期为21d,2个周期后评价短期疗效,平消片连服3个月以上。结果治疗组、对照组有效(完全缓解+部分缓解)率分别为67.5%(27/40例),45.0%(18/40例),治疗组1、2年生存率分别为75.0%、62.5%,中位生存期450d;对照组1、2年生存率分别为70.0%、30.0%,中位生存期368d,两组有效率及2年生存率有显著差异(P<0.05)。治疗组白细胞(WBC)下降占40.0%,血小板计数(BPC)下降占45.0%,对照组WBC下降占62.5%,BPC下降占67.5%。胃肠道反应恶心、呕吐发生率治疗组45.0%,对照组67.5%,两组有显著性差异(P<0.05)。结论平消片合用GP方案联合治疗中、晚期NSCLC,疗效显著,毒副作用减少,能增加机体免疫功能及抗癌能力,提高生存率和生存质量。Objective To observe the efficacy and adverse reactions of pingxiao tablets combined with GP（gemcitabine,cisplatin）chemotherapy and GP chemotherapy alone for the treatment of intermediate stage and advanced non-small cell lung cancer（NSCLC）.Methods Seventy advanced NSCLC patients were randomly divided into GP chemotherapy alone group（35 patients）and GP plus pingxiao tablets group（35 patients）.Pingxiao tablets were administed before and after GP chemotherapy.One treatment cycle was 21 days,the short-term effect was evaluated after 2 cycles treatment.The treatment of pingxiao tablet lasted for more than 3 months.Results The effective rates（CR ＋ PR）of the treatment were 65.7%（23/35 cases）and 48.6%（17/35 cases）in GP plus pingxiao group and GP group respectively.One year and 2-year survival rates were 80% and 54.2% respectively,median survival was 450 days in GP plus pingxiao treatment group.One year and 2-year survival rates were 73.3% and 33.3% respectively,median survival was 368 days in GP group.There were significant differences in efficient rate and 2-year survival rate between the two treatment groups（P〈0.05）.Decreased WBC and decreased BPC were found in 68.6% and 62.9% of the cases respectively in GP plus pingxiao treatment group;while they were 86.7% and 73.3% in GP group.There was significant difference（P〈0.05）in the incidence rate of nausea and vomiting（42.9% in GP plus pingxiao group and 60% in GP group）between the two groups.Conclusion Significant efficacy,reduced side effects and improved survival are obtained by GP chemotherapy combined with pingxiao in the treatment of NSCLC.
