腰椎管内巨大脂肪瘤的外科治疗

摘　　要：目的探讨腰椎管内脂肪瘤的临床特点及手术治疗效果。方法对4例腰椎管内巨大脂肪瘤的临床特点及影像学资料进行分析,经后正中切口入路,行椎板减压手术切除脂肪肿物。结果患者术后肌力均恢复正常。2例发生脑脊液漏。症状完全缓解3例,好转1例。4例均获随访,时间10个月～3年,脂肪瘤未见复发。结论对有椎管压迫症状的腰椎管内脂肪瘤应行手术治疗,行后路椎板减压能取得良好的临床疗效。Objective To investigate the clinical characteristics and surgical outcome of the lumbar spinal canal lipoma.Methods Clinical features and radiological materials of four patients with giant lumbar spinal canal lipomas were studied.Operation was done on each patient via the posterior laminotomy and surgical removal of lipomas.Result All patients,myodynamia returned to normal postoperatively.2 patients got leakage of cerebrospinal fluid.3 patients got syndrome totally improved,and 1 patient was partially improved.The follow-up period was from 10 months to 3 years in all 4 cases.None of the patient got recurrence.Conclusions Operation should be done in these patients who has spinal lesions,and laminotomy decompression is an effective method to treat the lipoma.
