超声引导移植肾穿刺活检的临床价值

摘　　要：目的探讨超声引导下经皮移植肾穿刺活检的可行性、安全性和临床应用价值。方法回顾分析170例在超声引导下进行移植肾穿刺活检患者的病例资料。结果 170例患者共行340次肾穿,取材成功率99.7%(339/340)。穿刺取材质量好为86.1%(292/339)。病理诊断急性排斥反应(acute rejection,AR)78例,急性肾小管坏死(acute tubular necrosis,ATN)9例,慢性排异反应(chronic rejection,CR)18例等。严重并发症发生率3.5%。结论超声引导的移植肾穿刺活检为高安全性和可行性的诊断手段。Objective To investigate the feasibility,safety and clinical value of ultrasound-guided transplanted kidney biopsy.Methods Retrospective analysis from 170 cases of ultrasound-guided transplanted kidney biopsy was made.Results Totally 340 times of biopsy procedure were conducted in all 170 cases,and the achievement ratio of drawing material was 99.7%（339/340）.The ratio of good quality of the specimens was 86.1%（292/339）.The pathological diagnosis was acute rejection in 78 cases,acute tubular necrosis in 9 cases,chronic rejection in 18cases,etc.Incidence rate of severe complications was 3.5%.Conclusion Ultrasound-guided transplanted kidney biopsy is a diagnostic method with high security and feasibility.
