全脑放疗联合立体定向分割放疗治疗脑转移瘤30例

摘　　要：[目的]探讨全脑放疗加立体定向分割放疗治疗脑转移瘤患者的疗效。[方法]30例1～4个脑转移瘤患者接受全脑放疗30~36Gy/（15~30f·3~3.5w）后加立体定向分割放疗25Gy/（5f·1周）（WBRT＋SRT组）。30例1～4个脑转移瘤患者接受单纯全脑放疗30Gy/（10f·2周）（WBRT组）。分析两组患者的1年局控率和1年生存率。[结果]WBRT＋SRT组和WBRT组1年局控率分别为76.3%、23.5%（P〈0.01）,1年生存率分别为46.7%、13.3%（P〈0.05）。两组均未出现严重毒副反应。[结论]全脑放疗加立体定向分割放疗（WBRT＋SRT）治疗脑转移瘤患者安全有效,可提高1～4个脑转移瘤病灶患者的1年局控率和1年生存率。[Purpose] To investigate the efficacy of whole brain radiotherapy（WBRT）combined with stereotactic radiotherapy（SRT）in the treatment of metastatic brain tumor.[Methods] Thirty cases with 1～4 brain metastases underwent WBRT（30～36Gy/15～30f·3～3.5W）plus SRT（25Gy/5f· 1W）and 30 cases with 1～4 brain metastases underwent WBRT alone（30Gy/10f·2W）.The 1-year survival rate and local control rate of the two groups were analyzed.[Results]The 1 year local control rate in WBRT plus SRT group and in WBRT alone group was 76.3%and 23.5%respectively（P0.01）.And 1-year survival rate was 46.7%and 13.3%respectively（P0.05）.No serious toxicity was found in both groups.[Conclusions]WBRT plus SRT for patients with metastatic brain tumor is feasible.It can significantly improve local control and 1-year survival rates in patients with 1～4 metastatic brain tumor.
