超声乳化联合玻璃体切割术治疗全葡萄膜炎继发青光眼

摘　　要：目的:评价超声乳化联合玻璃体切割术治疗全葡萄膜炎继发青光眼的临床疗效。方法:对全葡萄膜炎继发青光眼14例患者行超声乳化联合玻璃体切割术,术后随访3个月至3年,观察患者视力、眼压及合并症情况。结果:患者术后矫正视力高于术前矫正视力(P<0.05);术后7 d眼压(17.78±2.63)mmHg低于术前眼压(21.85±4.80)mmHg;术后10眼(71.4%)矫正视力优于术前最佳矫正视力;术后闪辉情况加重8眼(57.1%),在使用地塞米松+万古霉素球旁注射7 d后,闪辉情况得到控制;14例中均未出现玻璃体出血、新生血管性青光眼、黄斑囊样水肿。结论:超声乳化联合玻璃体切割术是一种行之有效、安全、经济且减少患者痛苦的治疗全葡萄膜炎继发青光眼的方法。Objective： To evaluate the therapeutic efficacy of vitrectomy combined with phacoemulsification on panuveitis with secondary glaucoma.Methods： Vitrectomy combined with phacoemulsification was applied to treat 14 panuveitis patients with secondary glaucoma.Following-up ranging from 3-36 months was conducted to observe the patients＇ visual acuity,intraocular pressure and complications.Results： The follow up showed postoperative corrected visual acuity were significant better than preoperative ones（P0.05）,especially,10 eyes（71.4%） showed corrected visual acuity better than that the best corrected visual acuity before the treatment.Seven days after the operation,IOP [（17.78±2.63） mmHg] was lower than that before the operation [（21.85±4.80）mmHg].Inflammatory deteriorated in 8 eyes（57.1%）,but were all under control after using dexamethasone and vancomycin for 7 days.No complications like vitreous hemorrhage,neovascular glaucoma,or cystoid macular edema were observed.Conclusions： Vitrectomy combined with phacoemulsification is a safe,effective,cost saving treatment for panuveitis with secondary glaucoma.
