摘  要:消化道溃疡尤其是幽门螺杆菌感染阳性患者,实验应激性胃溃疡以及非甾体类消炎药所致实验性胃溃疡,在粘膜损伤部位及其周围组织细胞生成并释放IL-1β、IL-6、IL-8、TNF-α、PAF及EGF等大量细胞因子,某些细胞因子间还形成相互制约相互作用的细胞因子网络,对溃疡的发生、发展及修复过程产生广泛影响。如IL-1β、TNF-α可通过诱导生成IL-8诱发细胞氧呼吸暴发产生氧自由基使溃疡加剧;PAF是内源性促溃疡形成因子中最强的一种,它强烈诱导中性粒细胞聚集,使炎症反应加剧,促进血小板聚集造成血栓形成,加重缺血时粘膜微血管堵塞造成粘膜下血管淤血及坏死,PAF拮抗剂可显著减轻和预防其粘膜损伤效应;而EGF有显著的胃粘膜保护作用已被用于消化道溃疡治疗,疗效显著。A large number of cytokines including IL-lβ, IL-6,IL-8, TNF-α, PAF, and EGF are formed and released from gastric ulcer area and its round mucosa tissue and cells, which are found in NSAIDs induced-gastric ulcer, stress induced-gastric ulcer and Helicobacter pylori infected gastric ulcer patient. Some of cytokines formed the cytokine network to produce widely effects on ulcer pathogenesis, development and ulcer healing. It is known that IL-8 have an active role in the induction of the cells respiratory burst, and IL-8 can be induced by IL-1β, and TNF-α. PAF is one of the most potent ulcer ogens of gastrointestinal tract, capable of inducing proinflammatory events such as cells chemotaxis, increase platelet agglutination and thrombosis, PAF antagonists have significantly preventive and therapeutic effects on PAF-induced gastric mucosa damage. EGF showed a very good therapeutic action and mucosa protective effect on the gastric ulcer.
