E-钙黏蛋白、β-连环蛋白在食管腺癌的表达及其意义

摘　　要：目的探讨食管腺癌患者上皮钙黏蛋白(E-cad)和β-连环蛋白(β-cat)表达与临床病理特征及预后关系。方法应用免疫组化方法检测51例食管腺癌组织中E-cad和β-cat的表达,术后对患者进行随访以了解生存情况,结合患者临床病理学特征、生存期进行统计学分析。结果在所有食管腺癌组织标本中,E-cad膜表达与腺癌分化程度、浸润深度、有无淋巴结转移有密切关系(P<0.05);β-cat蛋白异常表达与腺癌低级别分化相关(P<0.05);两种蛋白异常表达率均随临床分期进展而显著增加(P<0.05),E-cad、β-cat表达异常者生存期显著缩短,具有明显的死亡风险(PE=0.01,Pβ<0.001),但E-cad、β-cat并不能作为预测食管腺癌预后的独立因素。结论 E-cad和β-cat表达异常是判断食管腺癌浸润转移的重要指标,能够影响患者预后。Objective To observe the expression of E-cadherin and β-catenin in esophageal adenocarcinoma, and to uncover their correlations with clinicopathologic properties and prognosis of the patients. Methods The expression of E-cadherin and β-catenin in the 51 resected esophageal adenocarcinoma tissues was detected by immunohistochemical technique,and the pa- tients were followed up postoperatively for the survival situation, expression of the proteins was evaluated with respect to clinico- pathological properties and patient survival. Results Expression of E-cadherin correlated significantly with grade,invasive depth and metastasis of the carcinomas （ P 〈 0.05 ）. The abnormal expression of β-catenin correlated with lower grade （ P 〈 0.05 ） , the abnormality rates of both proteins increased with clinical staging. Abnormal expression of the two proteins correlated with shor- ter patient survival. Patients with the abnormal expression of E-cadherin and β-catenin undergo significantly higher mortality risk （ PE = 0.01, P 〈 0.001 ）, however, neither of them could be used as significant prognosticators for survival. Conclusion Abnormal expression of E-cadherin and β-catenin could be valuable indicators for predicting invasion and metastasis of esophageal adenocarcinoma, and important for prognosis of patients.
