摘  要:目的 探讨多层螺旋CT不同测量技术对评估125I粒子治疗肺癌疗效的一致性.方法 对30例肺癌患者进行125I植入治疗,于粒子植入前及植入3个月后进行CT扫描.分别用三维体积测量与一维直径、二维面积测量法在不同的时间2次测量肿瘤的尺寸,记录其平均值,计算治疗后的测量差异.根据WHO标准(二维)、RECIST标准(一维)及三维标准将肿瘤反应分为4类.用Kappa分析法分析3种测量方法评估肿瘤治疗反应的一致性.结果 一维及二维测量结果具有极好的一致性(k=0.862,P=0.000),一维和三维(k=0.395,P=0.030)、二维和三维(k=0.389,P=0.029)测量方法之间一致性均较差.结论 利用现有的疗效分类标准,三维体积测量同一维测量及二维测量在评估肺肿瘤治疗反应时有差异.Objective To study the efficacy consistency of lung cancer treatment with 125I seed using computed tomography(CT) different measurement. Methods Chest CT scans of 30 patients with lung cancers were performed before and 3 months after treatment with 125 I. The tumors were measured in volume twice pre and post- treatment with three methods including one- dimension (1D), two-dimension(2D) and three-dimension(3D) respectively, and the average values were respectively acquired, which were used to calculate the changes of the tumors in size after treatment. Depending on WHO (2D), RECIST(response evaluation criteria in solid tumors) (1D) and volumetric(3D) criteria,the treated response was categorized into four types. Kappa analysis was used to de- termine whether there was agreement between three methods. Results There was a good consistency between 1D and 2D method(k =0. 862,P=0. 000). Poor consistency was found between 1D and 3D (k=0. 395,P=0. 030),2D and 3D(k=0. 389,P=0. 029). Conclusion There are differences in evaluating the treatment response of lung cancer with 1D,2D and 3D measurement.
